Monthly reporting form

Monthly Power Plant Report," Form EIA-759 "Annual Electric Generator Report," Form EIA-860 "Annual Electric Utihty Report," EIA-861, Energy Information Administration, U.S. Department of Energy, Washington, D.C., 1993. [Pg.18]

At the end of each month, the field cost engineer collects all current information on a detailed cost report form. As these are actual costs, they can be used to estimate future job costs to completion. Daily reports of unit-cost progress for concrete, excavation, masonry, steel, piping, and electrical work, etc., are then used to predict possible overruns or underruns for the various items. Analysis and comparison with the original estimate point out trouble spots for early attention. If an item is running into difficulty, it is red-flagged to the resident and projec t engineers for remedial action. [Pg.875]

A. I assume that the statistical department compiled the monthly reports in one form or another. [Pg.222]

However, the SSC does not just report on security situations. The center also collects data provided by the appliances in the homes. These data are processed and given to the owners in the form of monthly reports. [Pg.238]

As implied by its title, the safety update report is not submitted with the original BLA, but is submitted in the form of updates at specific points in the application review process. Applicants must submit safety update reports 4 months after the BLA submission, after receipt of a complete response letter, and other times requested by CBER. In these reports, the sponsor must update the pending BLA with new safety information learned about the product that may reasonably affect the labeling statements in the contraindications, warnings, precautions, and adverse reactions sections. The updates must include the same types of information from clinical studies, animal studies, and other sources, and must be submitted in the same format as the BLA s integrated safety summary. They must also include case report forms for each patient who died during a clinical study or who did not complete the study because of an adverse event. [Pg.181]

They can be established for the qualifications of CRO personnel (e.g. a senior CRA must have at least 2 years of clinical research experience) timing and content of reports (e.g. the CRA s monitoring report must follow the format of the example given, and a copy of the report must be received by the sponsor within 2 weeks of the monitoring visit) patient enrollment (e.g. each site must enroll a minimum of 10 patients/month for the first 4 months of the study) cycle times (e.g. questions on case report form content, queries , must be generated within 1 week of receipt of the data by the CRO) database accuracy (e.g. the error rate as determined by comparing actual case report forms with the CRO database must be no more than... [Pg.479]

All the information, whether in accident books or report forms, will need to be analysed so that useful management data can be prepared. Many organizations look at analysis each month and annually. However, where there are very few incidents, quarterly may be sufficient. The health and safety information should be used alongside other business measures and should receive equal status. [Pg.339]

A question often asked is whether or not the reporter s name should be required on the near miss incident report forms and appear on the subsequent weekly or monthly summary. It may be that general publication of near miss incident reports could create peer pressure if employees realize the near miss incident information was to be widely publicized within the organization. If near miss incidents are widely reported, employees will be exposed to many reports and an additional report should not draw attention to the reporter, thereby minimizing peer pressure. [Pg.55]

The LME contract specifies refined pig lead of minimum 99-97 per cent purity for good delivery. The LME lead price was quoted in pounds sterling per long ton until 1969, when the standard weight was changed to metric tons. From July 1993, the price has been denominated in US dollars per metric ton. Prices are quoted daily for cash and three months metal, and these are widely reported. Futures prices are also available for up to 15 months forward. As well as its importance for the pricing of refined metal worldwide, the LME cash and three months quotations form the basis for sales of lead concentrates, scrap raw materials and intermediate products (see Chapter 16 and below). [Pg.201]

Plasmodium vivax, responsible for the most prevalent form of malaria (benign tertian), has an incubation period of 8—27 days (14 average). A variety seen in northern and northeastern Europe has an incubation period as long as 8—10 months. The disease can cause splenic mpture and anemia. Relapses (renewed manifestations of erythrocytic infection) can occur with this type of malaria. Overall, P. vivax is stiU susceptible to chloroquine however, resistant strains have been reported from Papua New Guinea and parts of Indonesia. Plasmodium malariae the cause of quartan malaria, has an incubation period of 15—30 days and its asexual cycle is 72 hours. This mildest form of malaria can cause nephritis in addition to the usual symptoms. It is a nonrelapsing type of malaria but the ted blood ceU infection can last for many years. No resistance to chloroquine by this plasmodium has been reported. Plasmodium ovale responsible for ovale tertian malaria, has an incubation period of 9—17 days (15 average). Relapses can occur in people infected with this plasmodium. No chloroquine resistance has been reported for this parasite. [Pg.270]

Most facilities keep records on the operating status of the plant, usually in the form of monthly status reports or a chart that displays production level versus days, weeks, or months over the plant life. Changes in plant status are generally noted by date on either of these two data sources, but may also be logged separately. This information is important so that an accurate count of the number of hours spent in each plant state (operating versus nonoperating) and number of demands due to plant state changes can be used for reliability and risk analysis. [Pg.215]

There is evidence for immunosuppressive effects of PAHs in rodents (Davila et al. 1997). For example, strong immunosuppressive effects were reported in mice that had been dosed with benzo[fl]pyrene and 3-methyl cholanthrene, effects that persisted for up to 18 months (Environmental Health Criteria 202). Multiple immu-notoxic effects have been reported in rodents, and there is evidence that these result from disturbance of calcium homeostasis (Davila et al. 1997). PAHs can activate protein tyrosine kinases in T cells that initiate the activation of a form of phospholipase C. Consequently, release of inositol triphosphate—a molecule that immobilizes Ca + from storage pools in the endoplasmic reticulum—is enhanced. [Pg.189]
