Surgical technique under general anesthesia, electrodes (Medtronic Model 3387 DBS lead Medtronic, Inc., Minneapolis, MN) are stereotactically placed in both left and right CM nuclei through a coronal incision and bifrontal burr holes made at a distance of 10 to 15 mm at each side of the midline at the level of the coronal suture. The CM localization is accomplished by air ventriculography. This method allows us to demonstrate the anterior commissure (AC) and posterior commissure (PC) of the third ventricle with remarkable precision. Two lines are drawn, the AOPC line and the vertical line perpendicular to the PC (VPC). The target point for the electrode tip was a distance 10 mm from the midline and the intersection of the AC-PC line with the VPC [Velasco et al, 1989,2000b]. [Pg.563]

In some cases, a buddy wire approach (Fig. 5.11) with two 0.014in. guide wires can be used to facihtate lead dehvery in acutely angled vessels. Alternatively, an acutely angled lead (such as the Medtronic Model 2187 unipolar lead) might be able to be manipulated into the proximal portion of the CS branch. [Pg.260]

As its name implies, a consecutive interval counting scheme requires that a certain number of ventricular beats occur consecutively and are classified to a tachycardia zone in order to satisfy detection criterion. This number is programmable. For example if the number of beats needed to detect is 10, then 10 consecutive tachycardia beats are required to satisfy detection. Each interspersed non-tachycardia beat causes counting to be reset to zero. It is common to find this method of counting in VT detection zones. Exceptions include Guidant ICD models that utilize an "X of Y" counting method in their VT zones of detection, and Medtronic models where the physician has the choice of a consecutive interval or an "X of Y" counting scheme in the "Fast" VT zone. [Pg.54]

The Medtronic Talent device is a stent graft constructed from a plain woven polyester fabric and a nitinol stent frame. The bifurcated device for the abdominal aorta has a bare suprarenal stent, but no hooks or barbs. The thoracic stent graft has been replaced by the Valiant device (see below). Early Talent devices had a rigid longitudinal bar that was meant to provide columnar support. However, this component signihcantly decreased the flexibility of the device, and so has been discontinued in more recent models. [Pg.653]

Not all active-fixation leads have an extendable-retractable screw. Some models have a fixed screw coated in mannitol which dissolves in blood prior to fixation (Guidant, Sweet Tip ). As discussed previously, a thin diameter cabled lead inserted through a steerable catheter has a fixed screw which is essentially covered by the catheter until it is attached to the endocardium (Medtronic SelectSecure Medtronic Inc., Minneapolis MN) (Fig. 1.19).
