Massive leak. It may be due to faulty operation of pressure relief valve, chemically induced failure of tank wall, puncture by a sharp object, or normal operation of pressure relief valve in case of fire.

But each time ice formed and slowed the leak, the internal pressure within the tank increased and blew the plug out. By 1 00 P.M., the massive leak of 90 minutes duration created a huge visible white vapor cloud estimated to be nearly 1,100 ft. long (330 m), 800 ft. (240 m) wide, and about 5 ft. (1.5 m) high. [11] Five minutes later the cloud ignited. The flame flashed back to the source, and a blowtorch-like flame shot beneath the tanks. 

Cast iron is undesirable (and prohibited by certain flammable liquid codes) because a brittle fracture can lead to a massive leak. Some months after this mistake, the unit received a loss prevention audit and as a result the pump bowls were replaced with ductile iron units. This same audit prompted the installation of remotely operated emergency shut-off valves around these pumps. 

NO flow of feedwater. NO level in drum. NO fuel flow. LICV closed. Loss of feedwater pressure. Massive leak. Fuel line closed. Loss of level in drum and explosion of drum by flame impingement on dry shell if flame continues. Same as above. Flame extinguished. No steam generation. 

All investigative reports establish that the there was a massive leak of liquefied flammable materials for over an hour. All reports state flammables were directly from Pump 2601 or the closely attached piping to Pump 2601, which transferred olefins (propylene rich) to the nearby Cardon refinery.
