Ciclopirox olamine (Loprox) is a pyridone derivative available for the treatment of cutaneous dermatophyte infections, cutaneous C. albicans infections, and tinea versicolor caused by Malassezia furfur. It interferes with fungal growth by inhibiting macromolecule synthesis.

The fungus Pityrospornm orhiciilare cau.ses an additional type, tinea versicolor. This organism, called Malassezia furfur in older literature, cau.ses yellow to brown patches or continuous scaling over the trunk and occasionally the legs, face, and neck. The affected areas may be identified by the inability to tan in the sun. 

Seborrheic dermatitis is associated with aberrant serum levels of essential fatty acids and the presence of Malassezia furfur. Borage oil containing 25% GLA used topically has been effective in the treatment of seborrheic dermatitis in children, but it does not affect the growth of Malassezia furfur (Tollesson et al., 1997). 

Sulconazole, an imidazole derivative with antifungal effectiveness, is used in the treatment of tinea cruris, tinea corporis, and tinea pedis caused by Trichophyton menta-grophytes, Epidermophyton floccosum, and Microspomm canis. It is also used in the treatment of tinea versicolor caused by Malassezia furfur. 

Terbinafine 1% cream or spray is applied twice daily and is effective in tinea corporis, tinea cruris, and tinea pedis. Terbinafine is less active against Candida species and Malassezia furfur, but the cream also can be used in cutaneous candidiasis and tinea versicolor. In European studies, oral terbinafine has appeared to be effective in treatment of ringworm, and in some cases of onychomycosis. The systemic use of terbinafine is discussed above. 

Tolnaftate (1% cream, solution and gel) is indicated for treatment of tinea pedis (athlete s foot), t. cruris (jock itch), or t. corporis (ringworm) due to infection with Trichophyton rubrum, T. mentagrophytes, T. tonsurans, Microsporum canis, M. audouini, and Epidermophy ton floccosum, and as treatment for tinea versicolor due to Malassezia furfur.

On rare occasions in children, nosocomial pneumonias may occur as a result of metastatic infection secondary to a bacteremia or fungemia, or primary infection at some distant site, or from contiguous spread. Respiratory tract symptoms, such as tachypnea and increasing oxygen requirements with or without roentgenogram evidence of pneumonia, are common in infants with fungemia resulting from Malassezia furfur.
