Local reporters

Spills are covered by a variety of federal, state, and local reporting requirements and substantial penalties can result to a company and its employees for failing to report certain spills. Initial release notification is usually required immediately or within 24 hours of the release and, in some cases, written follow-up reports are required. Some of the applicable legislation is listed below and Table 64.1 lists some of the major reporting requirements for chemical spills that are specified by these Acts. However, refer to Hoechst Celanese s procedures for plant-specific requirements. 

The Council encourages health care professionals to report actual and potential medication errors to national (e.g., FDA MedWatch Program and/or the USP Practitioners Reporting Network), internal, and local reporting programs.

Laboratory abnormalities of routine tests most commonly involve tests of liver function, and patients with acute Q fever may present with a clinical picture of acute hepatitis. Depending on the locale, reported elevations of aspartate aminotransferase, alanine transferase, or both, in the range of 2- to 3-fold higher than the upper limit of normal, are observed in 50% to 75% of patients, while elevation of the alkaline phosphatase is observed in 10% to 15% of patients. The total bilirubin can be expected to be elevated in 10% to 15% of patients with acute Q fever. The white blood cell count is usually normal the erythrocyte sedimentation rate is elevated in one third of patients.65 Mild anemia or thrombocytopenia may also be observed.

Section 304 (40 CFR 355.40) Facilities are subject to state and local reporting for accidental releases, in quantities equal to or greater than their reportable quantities (RQ), of extremely hazardous substances (EHS) or CERCLA hazardous substances (40 CFR 302, Table 302.4) under Section 304. n-Hexane, cyclohexane, acetone, and some of the other solvents discussed are CERCLA hazardous substances and have CERCLA RQ for spills (Table 14.10.5). 

Once admitted to PICU, these children are of course much more likely to receive the treatment they need, but they then face other hazards recently mapped in a number of studies. In The Netherlands, Snijders and colleagues reviewed 4846 incident reports from 3859 admissions to neonatal intensive care units, the first speciality based study of locally reported incidents in these units. Medication and equipment problems dominated the reports though diagnostic issues, which are not usually well reported, were also frequent (Table 4.4). Significant patient harm was described in 70 of these reports ( 2% of admissions) ... 

Self-reported staff reactions that would potentially be taken after the event may be stated as follows (1) keep the event to oneself, (2) report the event to one s leader or the doctor in charge, (3) report the event to the local reporting system, (4) write in the patient s case-record about the event, (5) inform the patient about the event and future risk, (6) ejq)lain to the patient that the event was eaused by one s mistake and (7) apologise about the event to the patient. 

It is difficult to assess conclusively the accuracy of these early reports. Even if each incident could have been observed, a physical impossibility, one could never check the veracity of, say, an interview. In many cases, one knows that the interview must be, partly at least, journalistic fabrication because it is too stereotypical to be true, but this is far from objective proof Nevertheless, on the basis of those incidents that were observed, interviews with people who were present at others (local reporters, photographers, deckchair attendants, etc.) and a careful check on internal consistency, some estimate of the main distortions can be made. Checks with the local press are particularly revealing. Not only are the reports more detailed and specific, but they avoid statements like all the dance halls near the seafront were smashed when every local resident knows that there is only one dance hall near the front. 

Recent work has demonstrated that spin probes can be incorporated into complex materials and used as local reporters of the polymer host. By a judicious choice of the polarity, size, and chemical structure of the probe, it is possible to explore specific regions of microphase-separated or self-assembled system, thus by-passing the often difficult synthetic procedures of preparing covalently bound spin labels. The site selected by the probes is based on the very same weak inter-molecular interactions (electrostatic, hydrogen bonding, metal coordination) that... 

When sulfur dioxide is released into the environment, the appropriate regulatory agency should be notified. Sulfur dioxide does not have a reportable quantity listed in 49 CFR 172.101. [1] In the event of a release however, all state, provincial, municipal, and/or local reporting regulations must be complied with. It is most important that the response groups in the area affected be notified as quickly as possible. These indicators typically represent performance of individual components of the barrier system and are comprised of operating discipline and management system performance. Indicators at this level provide an opportunity to identify and correct isolated system weaknesses. Tier 4 indicators are indicative of process safety system weaknesses that may contribute to future Tier 1 or Tier 2 PSEs. Tier 4 indicators can provide opportunities for both learning and systems improvement. Tier 4 indicators are intended for internal organizational use and also for local reporting.
