Except for levetiracetam and gabapentin, which are eliminated mostly unchanged by the renal route, AEDs are metabolized wholly or in part by hepatic enzymes. [Pg.602]

Levetiracetam s activity may be related to its binding to the synaptic vesicle protein SV2A. [Pg.607]

Levetiracetam, topiramate, and the general anesthetics, halothane, isoflu-rane, and ketamine are being evaluated for refractory GCSE, but their efficacy, safety, or overall suitability have not been established to date. [Pg.659]

Levetiracetam Dosing Adjustment Regimen for Patients with Impaired Renal Function ... [Pg.1232]

Pharmacology Levetiracetam is chemically unrelated to other antiepileptic drugs. The precise mechanism by which levetiracetam exerts its antiepileptic effect is unknown. [Pg.1232]

Pharmacokinetics Absorption is rapid, with peak plasma concentrations occurring in approximately 1 hour following oral administration in fasted subjects. Steady state is achieved after 2 days of multiple twice/day dosing. Levetiracetam is not extensively metabolized. The plasma half-life in adults is approximately 7 hours. Levetiracetam is eliminated from the systemic circulation by renal excretion. [Pg.1232]

Somnolence - In controlled trials of patients with epilepsy, 14.8% of levetiracetam-treated patients reported somnolence compared with 8.4% of placebo patients. [Pg.1233]

Coordination difficulties - A total of 3.4% of levetiracetam-treated patients experienced coordination difficulties (reported as either ataxia, abnormal gait, or incoordination) vs 1.6% with placebo. [Pg.1233]

In addition, 4 (0.5%) levetiracetam-treated patients attempted suicide vs 0% with placebo. [Pg.1233]

Withdrawal seizure Withdraw levetiracetam gradually to minimize the potential of increased seizure frequency. [Pg.1233]

Renal function impairment Take caution in dosing patients with moderate and severe renal impairment and patients undergoing hemodialysis. Reduce dosage in patients with impaired renal function receiving levetiracetam, and give supplemental doses to patients after dialysis (see Actions and Administration and Dosage). [Pg.1233]

Elderly No overall differences in safety were observed between subjects 65 years of age or older and younger subjects. There were insufficient numbers of elderly subjects in controlled trials of epilepsy to adequately assess the efficacy of levetiracetam in these patients. Because elderly patients are more likely to have decreased renal function, take care in dose selection it may be useful to monitor renal function. [Pg.1233]

Lactation Levetiracetam is excreted in breast milk. Because of the potential for serious adverse reactions in nursing infants, decide whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the... [Pg.1233]
