Method validation has received considerable attention in the literature and from industrial committees and regulatory agencies. The international standard ISO/ IEC [1] requires validation of nonstandard methods, laboratory designed/devel-oped methods, standard methods used outside their intended scope, and amplifications and modifications of standard methods to confirm that the methods are suitable for their intended use. Adding to this problem is the lack of unambiguous, scientifically valid guidelines from the International Conference on Harmonization (ICH) guidance (1). This leads to having to choose between what the user thinks the regulators want and the scientific approach.

Several new developments on QT/QTc prolongation assessment have occurred recently. The most important includes the publication of the new International Conference of Harmonisation (ICH), Step 4 guidance, issued on May 12, 2005 entitled The Clinical Evaluation of QT/QTc Interval Prolongation and Proarrhythmic Potential for Non-Antiarrhythmic Drugs. It contains relevant information obtained from the ICH Steering Committee, as well as information discussed at the Qctober 2003 Food and Drug Administration (FDA) and Drug Information Association (DIA)... 

This chapter focuses on pharmacometric methods used in QT data analysis. Phar-macometric issues in the design and analysis of thorough QT clinical trials (TQT), and the methods used to assess QT prolongation potential are described. Analysis methods discussed are patterned after the new International Conference on Flar-monisation (ICH) guidance for clinical QTc assessment and are illustrated with data from real clinical trials. Issues in the interpretation of data are also discussed. 

Two useful books on dose-finding in clinical medicine are that edited by Ting (2006) and the one in this series edited by Chevret (2006). In the latter, the chapter by Hemmings (2006) provides a regulatory perspective. Regulatory guidance is provided by International Conference on Harmonization (1995). As regards practical implementation of dose escalation schemes in phase I in cancer, such as are discussed in Sections 20.2.7 to 20.2.9, some freely available software.
