Retinoids cause a variable amount of irritation, more properly a heightening of cutaneous reactivity termed retinoid dermatitis, early in therapy [39-42,44]. In the first few weeks approximately 80% of patients experience stinging, hyperesthesia or pruritus, fine scaling, dryness and mild erythema (Fig. 7). These effects are dose-dependent and peak within the first month of therapy. Weiss et al. reported 92% incidence with 0.1% tretinoin and Leyden et al. a 70% incidence using 0.05% tretinoin. However, in a double blind, vehicle controlled study of 179 patients, only 4% of patients using Renova discontinued the medication as a result of adverse reactions [57]. Discussion It is important that the apparatus assigned to the companies and personnel of this department be available to answer any and all alarms to ensure the safety of this community. Phenytoin may be administered either orally or intravenously and is absorbed slowly after oral administration, with peak plasma levels achieved after 3 to 12 hours. It is extensively plasma protein bound ( 90%), and the elimination half-life is between 15 and 30 hours. These large ranges reflect the considerable variability observed from patient to patient. Parenteral administration of phenytoin is usually limited to the intravenous route. Phenytoin for injection is dissolved in a highly alkaline vehicle (pFI 12). This alkaline vehicle is required because phenytoin is weakly acidic and has very poor solubility in its un-ionized form. Reportedly, however, its phosphate ester fosphenytoin has water solubility advantages over phenytoin for injection. Intramuscular phenytoin generally is avoided, because it results in tissue necrosis at the site of injection and erratic absorption because of high alkalinity. In addition, intermittent intravenous infusion is required to reduce the incidence of severe phlebitis. [Pg.1090]
