I Methadone

Methadone, in general, exerts a pharmacological action similar to that of morphine. Its action on smooth muscle resembles that of meperidine. analgesic potency, but /-methodone is twice as effective. When used in comparable analgesic doses, methadone is as toxic as morphine and exerts the same effects on the pulse rate and respiration as does morphine. Methadone exerts no sedative effect and, hence, is of no value as a preanesthetic agent.

Paroxetine 20 mg/day, a selective CYP2D6 inhibitor, was given for 12 days to 10 patients on methadone maintenance (42). Eight were genotyped as CYP2D6 homozygous extensive metabolizers and two as poor metabolizers. Paroxetine increased the steady-state concentrations of i -methadone and 5-methadone, especially in the extensive metabolizers.

Cannabis The combination of cannabis with methadone has been studied in 77 Australian and 74 Swiss methadone maintenance patients [98 ]. There were lower 24-hour dose-corrected trough plasma concentrations of both (i )-methadone and (S)-methadone there was no effect of sex, alcohol, tobacco smoking, or duration of methadone treatment.

LAAM (L-a-acetylmethadol or levomethadyl acetate) is a full agonist at the i opioid receptor with pharmacologic properties similar to those of methadone. A number of studies have demonstrated that treatment with LAAM results in reduction of opioid use and beneficial effects comparable to those achieved with methadone (Ling et al. 1978 Tennant et al. 1986 Zangwell et al. 1986). However, retention rates are higher in patients who take methadone doses of 80—100 mg/day.

Methadone has regained prominence in treating cancer pain. It has a prolonged mechanism of action, N- met hy 1 -1) - a s pa rl a I c receptor antagonist activity (d-isomer), and is inexpensive. However, it can be difficult to titrate.

Fernandez et al. [27] applied HPLC with diode array detection (DAD) to the determination of heroin, methadone, cocaine and metabolites in plasma after mixed-mode SPE. Analytes were separated using a RP8 column (250 mm x 4.6 mm i.d., 5 jam particle size) and acetonitrile-phosphate buffer pH 6.53 as mobile phase with elution in the gradient mode. The method, which provides a LLOQ of 0.1 j,g/mL for all compounds, was successfully applied to 21 plasma samples from fatal overdoses. 

I would look at putting her medication up. Again I would ask if there was anyone else in the family [to take care of the children] but the way I m looking at it, this ten-year-old seems to be doing a not too bad a job at lifting the baby and stuff like that and it might be okay for a couple of days until this new methadone kicks in. I don t think I would feel the need to take the baby into care straight away. 

SWl I think she s done well if she s been on methadone for a year to keep things going then. With a boy age ten and a baby girl of six months, she should be commended for that, I think.
