If a consumer is exposed to hazardous product characteristics, the severity level or potential consequence of this exposure must be evaluated. Human factors analysis is conducted to determine the consequences (i.e., potential product-related injuries) based on the foreseeable behaviors consumers will use when interacting with products. Virtual and physical models of the human anatomy are used to effectively diagnose and demonstrate hazardous product characteristics. (In contrast to a physical hazard such as those noted above, physical in this human context relates to the usage of three-dimensional (3D) models of various parts of humans relevant to the exposures associated with use and/or misuse of a product.)... 

Human factors analysis utilizes accurate virtual and physical simulations of the human anatomy to identify the potential hazards posed by consumer products. 

As we have stated and demonstrated repeatedly throughout this chapter, the technology for modeling human performance in systems is evolving rapidly. Furthermore, the breadth of questions being addressed by models is constantly expanding. We encourage the human factors practitioner with a little creativity and computer savvy to consider how computer simulation can provide a better and more cost-effective basis for human factors analysis.

The proper acquisition of relevant information about a task in a safety-critical environment is the foundation of every sound human factors analysis. Retain a firm with comprehensive PSM expertise. This firm should be an expert in pressure relief systems, safety instrumented systems, human factor analysis, etc. The firm must perform process safety audits, to conduct a refinery-wide comprehensive audit and analysis of the company s PSM systems [22]. 

In most industries the investigation of an incident, accident or near-miss starts as soon as possible after the event (within minutes or hours) to ensure that the evidence of what has happened is still fresh. When human factors analysis is conducted this often occurs after the main evidence collection phase of the investigation, and so may miss some of the more volatile evidence that is only available for a short time after the event. 

The following sections describe an approach developed by The Keil Centre based upon three existing human factors analysis tools. The tools are described first, followed by a description of the approach taken to integrate them for use by incident investigators. 

The use of RCA to draw together these human factors analysis tools in this way requires some form of procedure to be written around the associations between the RCA technique itself and the tools. Such a procedure needs to provide clear steps for the identification of the behaviours involved in the incident, and guidance on the classification of those behaviours as intentional or unintentional. This leads the analyst into the appropriate form of analysis, leading to the development of appropriate corrective actions. 

This paper focuses on the use of Human Factors analysis techniques in the investigation of incidents, and therefore concentrates on the retrospective application of these techniques. It is just as important to put in place prospective... 

Taking into account the significance of human factors to SoS safety, we have identified important roles for human factors analysis in both the lAT SoS hazard identification and hazard analysis processes. 

Apart from the reference provided, no guidance is given in either SEMS or SEMP (or in the OSHA Process Safety Management standard) as to how human factors analysis and management is to be implemented. 

The above statement provides no guidance as to the scope of a human factors analysis, when it is to be done, or how it links to the other elements of SEMP.
