Evaluation Service Report

Post-installed anchors must satisfy certain qualification requirements in order to be used to resist seismic loads in a structure assigned to Seismic Design Categories C, D, E, or F. These involve the performance of specific tests and application of acceptance criteria for qualification and determination of relevant design parameters. ACl 318 Appendix D references ACl 355.2, Qualification of Post-installed Mechanical Anchors in Concrete, for the quahfication of expansion and undercut anchors. This document has been incorporated into acceptance criteria used by the ICC Evaluation Service for issuance of Evaluation Service Reports on anchors to demonstrate conformance with IBC Section 104.11, Alternative materials, design and methods of construction and equipment. AC 193 provides acceptance criteria for mechanical anchors. AC308 provides acceptance criteria as well as supplementary design provisions for adhesive anchors. 

You should set up a common entry point for such data and put in place an evaluation function to convey appropriate data to the manufacturing, engineering, and design activities. A corrective action form or improvement form could be used to convey the data and obtain a written response of the action to be taken. A log of servicing reports would assist in tracking servicing concerns and demonstrate you were making effective use of the data.

In conjunction with its research activities, TPRC screens the world s literature and collects published information on a wide range of materials in the field of thermophysics. This information concerns data, theoretical estimation methods, and experimental measurement techniques. Technical papers come from journals, abstracting services, reports, doctoral dissertations, masters theses, and many other sources. The full evaluation and analysis of the collected raw data are needed before publications on recommended values can be prepared. Such effort is obviously time consuming and expensive and therefore this critical evaluation is currently performed at a rather modest funding level. The end result is that much of the available world literature is not being processed and distilled. 

Post-exposure prophylaxis, when medically indicated as recommended by the U.S. Public Health Service, shall be made available, as weU as counseling and evaluation of reported illnesses. 

Information regarding the anchor length, minimum edge distance, anchor spacing, and member depth are contained in evaluation reports issued by ICC-ES, or other evaluation services and in product literature. Design for reduced anchor spacing and edge distance is addressed in the provisions of ACI 318 Appendix D.

Evaluation data taken from the more than 8 thousand reports of the laboratory and field wire ropes inspections show that the visual methods and life time criteria are non adequate to real durability of the ropes in service [4]. The data show that only a very small percentage of all ropes was replaced in a proper time, when one has used a visual inspection.

The review of the data portion of the Indian Point 2 (IP2) and 3 (IPS) PRA (a 1982 internal document prepared by Consolidated Edison and the New York Power Authority) is confined to the plant-specific and generic component failure and service hour data sections because these were the only segments available to the reviewers. The LERs produced during a ten-year span of IP2 s operation were evaluated to determine their applicability to the PRA data needs. It was eventually decided to use only the LERs generated after IP2 became critical (from May 23, 1973 to December 31, 1979) for the component data base development, based on the availability of failure event information and more uniform operability, testing, and reporting criteria.
