The chemical constituents of interest for cognitive effects are the ergotoxine derivatives, otherwise referred to as ergoloid mesylates (Schneider and Olin 1994). Ergotoxine was isolated in 1906, but later discovered to be a mix of four alkaloids ergocristine, ergocornine, o-ergokryptine.

Pharmacoiogy Ergoloid mesylates contain equal proportions of dihydroergocornine mesylate, dihydroergocristine mesylate, and dihydroergocryptine mesylate. 

The mechanism by which ergoloid mesylates produce mental effects is unknown. Pharmacokinetics Ergoloid mesylates are rapidly absorbed from the Gl tract peak plasma concentrations are achieved within 0.6 to 3 hours. The liquid capsule has a 12% greater bioavailability than the oral tablet. The mean half-life of unchanged ergoloid in plasma is about 2.6 to 5.1 hours. 

Before prescribing Before prescribing ergoloid mesylates, exclude the possibility that the patient s signs and symptoms arise from a potentially reversible and treatable condition. Exclude delirium and dementiform illness secondary to systemic disease, primary neurological disease, or primary disturbance of mood. Reassessment Periodically reassess the diagnosis and the benefit of current therapy to the patient. 

Contraindications Acute or chronic psychosis (regardless or etiology), hypersensitivity to ergoloid mesylates or any component of the formulation... 

Dihydroergotoxine, a mixture of dihydro-K-ergocryptine and three similar dihydrogenated peptide ergot alkaloids (ergoloid mesylates), has been promoted for many years for the relief of senility and more recently for the treatment of Alzheimer s dementia. There is no useful evidence that this drug has significant benefit. 

A group of drugs known collectively as ergot derivatives display some alpha-blocking ability as well as other unique properties. Ergot alkaloids and ergoloid mesylates and are used clinically for diverse problems, including the treatment of vascular headache and improvement of mental function in pre-senile dementia.

Thirteen subjects were given 0.5 g/kg of alcohol 25% in orange juice after breakfast, before and after taking 4.5 mg of codergocrine mesilate (ergoloid mesylates, Hydergine) every 8 hours for nine doses. The codergocrine caused a small reduetion in blood-alcohol levels (maximum serum levels reduced from 59 mg% to 55.7 mg%), and the clearance was reduced by a modest 11%. The reason is not understood. This interaction is almost certainly not of clinieal importance.
