Chapter 1, The Role of Human Error in Chemical Process Safety, discusses the importance of reducing human error to an effective process safety effort at the plant. The engineers, managers, and process plant personnel in the CPI need to replace a perspective that has a blame and punishment view of error with a systems viewpoint that sees error as a mismatch between human capabilities and demands. 

A panel of experts from academia and industry (Table 12.4-2) was assembled to provide the students with professional advice. The HKUST central facilities for Material Characterization and Preparation Facility (MCPF) and Advanced Engineering Material Facility (AEMF) and the laboratories of Department of Chemical Engineering, Department of Industrial Engineering and Engineering Management, Department of Mechanical Engineering and Advanced Technology Center play a pivotal role in the success of the project. Table 12.4-3 lists the equipment used by the students in this project.

They almost always need coaching in the interpersonal skills, and broader leadership skills, required for success in their roles. I have never seen an engineering manager fail because he/she was a bad engineer 1 have seen some fail because they were bad at dealing with people and inspiring followership. 

One of the roles of engineering managers is to set up processes that minimize the likelihood of design engineers making mistakes. 

On Friday 9th March 1982, I had been working on shift as an Assistant Shift Manager on the steam side - i.e. the turbo-generator and boiler side of the plant -for maybe six months. I had still only been working at PFR for about 2.5 years, which I am sure most power station engineers would say is not enough time to be in a shift operations management role.
