Diabetes mellitus monitoring

Thiazides cause a loss of sodium, potassium, and magnesium and promote calcium reabsorption. They affect glucose tolerance and should be used with caution in clients with diabetes mellitus. Monitor laboratoiy tests for electrolytes and glucose.

Diabetes mellitus A /o d use of amiloride in diabetic patients. If it is used, monitor serum electrolytes and renal function frequently. Discontinue use 3 days or more before glucose tolerance testing. 

Renal function /mpa/rmenf Anuria, acute or chronic renal insufficiency and evidence of diabetic nephropathy are contraindications because potassium retention is accentuated and may result in the rapid development of hyperkalemia. Do not give to patients with evidence of renal impairment (BUN greater than 30 mg/dL or serum creatinine greater than 1.5 mg/dL) or diabetes mellitus without continuous monitoring of serum electrolytes, creatinine, and BUN levels. 

Adverse reactions occur frequently. Rapid drug infusion may produce tachycardia, vomiting, shortness of breath, headache, and a fall in blood pressure. Changes in blood sugar (hypoglycemia or hyperglycemia) necessitate caution in its use, particularly in patients with diabetes mellitus. Renal function should be monitored and blood counts checked for dyscrasias.

Blood glucose should be monitored in patients with diabetes mellitus who receive glucose-containing intravenous immunoglobulin (484). 

Protease inhibitors can cause a rise in blood glucose concentration, although only a few cases have been reported. Patients with a family history of diabetes mellitus may be at a greater risk, and they demand especially close monitoring, for example with both baseline and quarterly glucose determinations, at least during the first 6-12 months of treatment (975,976).
