Recent human and animals studies have found that dextromethorphan reduces the intensity of post-surgical pain [1-4]. A meta-analysis of preemptive administration of dextromethorphan for acute post-operative pain management demonstrated proof of efficacy (reduction in opioid requirements) [1]. A study performed by Henderson and coworkers [4] found that dextromethorphan reduces post operative pain intensity and morphine dose requirements in patients recovering from abdominal hysterectomy. Fifty patients were randomized to receive a preemptive oral dose of dextromethorphan 40 mg, then 40 mg three times per day for the next 2 days, or placebo solution at identical times. Median pain scores at rest were significantly lower at 48 and 72 hrs and mean IV-PCA morphine consumption was reduced in the dextromethorphan group (1.1 vs 1.5 mg/h) P = 0.54. 

Chia and coworkers [5] evaluated the effectiveness of dextromethorphan in 60 patients recovering from major abdominal surgery. Thirty patients received an IV infusion of dextromethorphan 5 mg/kg before anesthetic induction (Pre group), whereas the remaining 30 patients received the same volume of saline solution, followed by a post-operative IV infusion of dextromethorphan 5 mg/kg (Post group).
