Detoxification short-term

Even in a short-term detoxification, an initial stabilizing period on the same methadone dose is often desirable, so that the user gains confidence in the treatment, and the effects of cessation of other drugs can be gauged before dose reduction. Further counselling on the withdrawal process can be done in this period, as typically some aspects do not sink in while negotiation to secure methadone treatment is still under way. 

Detoxification with tranquillisers, and avoiding the use of controlled drugs. This method is not very readily chosen and can be problematic in holding the patient in therapy. It can be useful for those who have been partly withdrawn as outpatients and who wish to come into hospital for the last period of detoxification. The use of thioridazine or chlorpromazine can be considered along with benzodiazepines in the short term. 

The term flexible prescribing has not yet been adequately defined, but would seem to imply first, individually-tailored treatment responses based on assessment of each client s needs and abilities, particularly the extent to which they are currently able or willing to control their drug use second, acceptance that short-term detoxification contracts may fail repeatedly and may need replacing... 

For short-term detoxification, a person would decrease and stop methadone in less then a month. It should be noted that people in short-term detoxification programs have a higher likelihood of returning to heroin abuse than do people in a long-term methadone detoxification program.

The conventional drug therapy for opioid withdrawal has been methadone, a synthetic opiate. Recently, buprenorphine has been approved for opioid withdrawal and will be discussed in detail below. In detoxification treatment, methadone is administered in decreasing doses over a period not exceeding 30 days (short-term detoxification) or 180 days (long-term detoxification). Most (62.5 percent) of public-sector treatment occurs in ambulatory settings. This means that the client doesn t spend 24 hours a day in the facifity. Over half of the admissions were ambulatory outpatient (1 to 2 hours, 1 to 2 days a week), nearly 11 percent were intensive outpatient (2 to 6 hours a day, 3 to 5 days a week), and almost 2 percent were detoxification (a period of abstinence from alcohol and other drugs to clear the system). Over 17 percent of admissions were in residential or rehabilitation facilities, where 24-hour care is provided. Of the admissions to residential or rehabilitation facihties, about half were short-term (less than 31 days) and about half were long-term (more than 31 days). One percent of the admissions were in a hospital. Over 20 percent of the admissions were in 24-hour service detoxification facihties. ... 

Detoxification is not really treatment. It simply is a process to get a person cleaned up. Detoxification clients are certainly referred to treatment but many go back out on the street following detoxification. So, if you remove detoxification from the treatment data, nearly two-thirds of actual treatment occurred in an ambulatory, outpatient setting which is the least intensive treatment nearly 13 percent in intensive outpatient, about 11 percent was short-term residential, and 11 percent was long-term residential. 

Persons under 18 years of age are required to have had two documented attempts at short-term detoxification or psychosocial treatment within 12 months to be eligible for maintenance treatment. A parent, legal guardian, or responsible adult must sign the consent form for the person unless they are an emancipated minor. Age appropriate treatment is a requirement. 

Short-term detoxihcation is detoxification with methadone for a period of time less than 30 days. Long-term detoxification is detoxification with methadone for a period of time more than 30 days but less than 180 days. Patients who are unsuccessful for two attempts must consider alternative treatment. Detoxification does not address the social, psychological, or behavioral problems associated with addiction. Short-term detoxification patients are not eligible for take-home medication however, long-term detoxification patients are eligible.

Clomethiazole should not be given long-term for alcohol withdrawal states or to those who continue to drink alcohol. Use for more than 9 days is not recommended. It has been said that if prescribers choose to manage detoxification at home, it should be done under very close supervision, issuing prescriptions for only one day s supply to ensure daily contact and to minimise tiie risk of abuse. And if the patient shows evidence of tolerance or clomethiazole dependency or of continuing to drink alcohol, the only safe policy is rapid admission for inpatient care. The manufacturer warns that alcohol combined with clomethiazole particularly in alcoholics with cirrhosis can lead to fatal respiratory depression even with short-term use. ... 

In all of these test systems, as also in experimentation with animals, the problem of extrapolation to man is always present. In the case of short-term tests, we are generally extrapolating from an effect on more or less naked DNA or, in some cases, a single cell system devoid of the controls and detoxification mechanisms one would find in the whole animal. 

Nonapparent plants were fast growing, short-lived, and occurred unpredictably in space and/or time. Thus, many of these plants would escape detection by the majority of herbivores. Because these plants allocated most of their resources to rapid growth and reproduction, they were believed to be defended by relatively low concentrations of toxic compounds that were generally effective against many herbivores but that some specialist herbivores would be able to evolve detoxification mechanisms against (and may even use the qualitative defenses as cues to locate their host). These toxic compounds such as alkaloids and terpenes were termed qualitative defenses because their potency made them effective at low concentrations qualitative defenses of the PAM are analogous to mobile defenses of the RAM. 

Liver. The liver performs a wide variety of chemical reactions in the body and is the main locus of detoxification. Successful liver transplantation is somewhat rare, and no true artificial liver seems likely in the near future. The process of hemoperfusion, which is sometimes termed an artificial liver, can be used to supplement or relieve the normal liver functions for short time periods. In this technique, the patient s blood is passed through a column or bed of some sorbent material that removes toxic chemicals from the blood. This technique is often used in cases of drug overdose, poisoning, and acute hepatitis. The sorbent material can be charcoal, ion-exchange resins, immobilized hepatic material, or liver material enclosed in artificial cells (microcapsules, usually made from a polyamide). The column is usually a plastic material, and plastic tubing is used to direct the blood flow to and from the device ( 1, 57, M).
