Even in a short-term detoxification, an initial stabilizing period on the same methadone dose is often desirable, so that the user gains confidence in the treatment, and the effects of cessation of other drugs can be gauged before dose reduction. The conventional drug therapy for opioid withdrawal has been methadone, a synthetic opiate. Recently, buprenorphine has been approved for opioid withdrawal and will be discussed in detail below. Detoxification is not really treatment. It simply is a process to get a person cleaned up. Detoxification clients are certainly referred to treatment but many go back out on the street following detoxification. So, if you remove detoxification from the treatment data, nearly two-thirds of actual treatment occurred in an ambulatory, outpatient setting which is the least intensive treatment nearly 13 percent in intensive outpatient, about 11 percent was short-term residential, and 11 percent was long-term residential. [Pg.130]

Persons under 18 years of age are required to have had two documented attempts at short-term detoxification or psychosocial treatment within 12 months to be eligible for maintenance treatment. Clomethiazole should not be given long-term for alcohol withdrawal states or to those who continue to drink alcohol. Use for more than 9 days is not recommended. It has been said that if prescribers choose to manage detoxification at home, it should be done under very close supervision, issuing prescriptions for only one day s supply to ensure daily contact and to minimise tiie risk of abuse. And if the patient shows evidence of tolerance or clomethiazole dependency or of continuing to drink alcohol, the only safe policy is rapid admission for inpatient care. Liver. The liver performs a wide variety of chemical reactions in the body and is the main locus of detoxification. Successful liver transplantation is somewhat rare, and no true artificial liver seems likely in the near future. The process of hemoperfusion, which is sometimes termed an artificial liver, can be used to supplement or relieve the normal liver functions for short time periods. In this technique, the patient s blood is passed through a column or bed of some sorbent material that removes toxic chemicals from the blood. This technique is often used in cases of drug overdose, poisoning, and acute hepatitis. The sorbent material can be charcoal, ion-exchange resins, immobilized hepatic material, or liver material enclosed in artificial cells (microcapsules, usually made from a polyamide). The column is usually a plastic material, and plastic tubing is used to direct the blood flow to and from the device ( 1, 57, M). [Pg.549]
