Danazol angioedema

Danazol is a weak androgen and also has a series of other hormonal and anti-hormonal properties. It inhibits pituitary gonadotropin and has been used in the treatment of endometriosis, fibrocystic disease of the breast, idiopathic thrombocytopenic purpura, and hereditary angioedema. Its hepatotoxic effects include reversible rises in serum transaminases and cholestatic hepatitis a few cases of hepatocellular tumors have been reported. 

A 34-year-old woman who had taken danazol 400 mg/ day for 13 years for hereditary angioedema developed a mass in the right hypochondrium. Her alcohol intake was under 20 g/day. She had a large heterogeneous hepatic tumor, a well-differentiated hepatocellullar carcinoma in a non-cirrhotic liver. 

Hereditary angioedema, with deficiency of Cj-esterase inhibitor (a complement inhibitor), may not respond to antihistamines or corticosteroid but only to fresh frozen plasma or preferably Cj-inhibitor concentrate. Delay in initiating the treatment may lead to death from laryngeal oedema (try adrenaline (epinephrine) i.m. in severe cases). For long-term prophylaxis an androgen (stanozolol, danazol) can be effective. 

Hereditary angioedema (lack of inhibition of the complement Cl esterase) may be prevented by androgens (stanozolol and danazol are used). 

Danazol is a sex hormone that suppresses the pituitary-ovarian axis by inhibiting output of pituitary gonadotropins has weak, dose-related androgenic activity with no estrogenic or progestational activity. Danazol is indicated in treatment of endometriosis symptomatic treatment of fibrocystic breast disease and in prevention of attacks of hereditary angioedema. 

Angioedema is caused by hereditary impairment of Cl -esterase inhibitor or acquired development of antibodies against it. The I7a-alkylated androgens, such as stanozolol and danazol, stimulate hepatic synthesis of the esterase inhibitor and thereby decrease the frequency of acute attacks. A 69-year-old woman with hereditary angioedema type I used danazol 200 mg three times a week and had only occasional crises requiring tranexamic add. She developed breast cancer and took tamoxifen, after which the severity and frequency of episodes of angioedema rapidly increased, despite maintenance of her usual treatment.
