The failure events were obtained from Savannah River Plant sources such as Reactor Incident Reports, daily logs, and operating summaries. The records include over 4,400 events going back to 1970 for an effective average of 3 operating nuclear reactors. Some entries represent data averaging about 110 reactor years experience accumulated since operation began. 

A boiler inspection includes a thorough examination of all waterside and fireside surfaces, plus the permanent boiler log, daily log, and other water treatment and maintenance records. Any economizers, attemperators, FW heaters, and other associated equipment may also be inspected.

The use and benefits of regression analysis can be appreciable, particularly in the evaluation of process data. This analysis in many cases has led to qualitative and often to quantitative determination of key operating variables whose effect had been masked on individual data point comparisons by the simultaneous changes in other less important, but unknown, variables.

The OSHAct requirements specify that any illness or injury to an employee that causes time lost from the job, treatment beyond first aid, transfer to another job, loss of consciousness, or an occupational iUness must be recorded on a daily log of injuries and illnesses, the OSHA 300 form (previously the 200 form). This log identifies the injured person, the date and time of the injury, the department or plant location where the injury occurred, and a brief description of the occurrence of the injury, highlighting salient facts such as the chemical, physical agent, or machinery involved and the nature of the injury. An injury should be recorded on the day that it occurs, but this is not always possible with MSDs and other cumulative trauma injuries. The number of days that the person is absent from the job is also recorded upon the employee s return to work. It is also possible to determine the severity of company injuries. Two methods are typically used. In the first, the total number of days lost due to injuries is compiled from the OSHA 300 daily log and divided by the total number of injuries recorded on the OSHA 300 daily log. This gives an average number of days lost per injury. In the second, the total number of days lost is multiphed by 200,000 and then divided by the number of hours worked by the company employees. This gives a severity index per 100 person hours of work. These measures can also be compa to an industry average. 

Keep a Record. It will benefit you to keep a daily work log. It includes a description of your assignment and how the assignment is fulfilled. You may add any thoughts on your work of that day. It is like a diary but devoted to the URE only. The daily log shall follow an overview describing your responsibilities and the project you are in. At the conclusion of the URE period, you can add a summary on your overall experience. Such a record will come in handy when you apply for graduate study and assistantship and when you apply for a job. 

The appointed person must sign the daily log certificate kept beside each machine which is an indication that the press is fit for use. Without the signed certificate, the press cannot be operated.

Second, the exceptions normally only exempt the driver from specific portions of the hours-of-service regulations, such as having to complete a daily log. They generally do not exempt the driver from all hours-of-service regulations and they certainly do not exempt drivers from other regulations, such as the requirement to carry a medical card. 

All drivers are expected to submit accurate, true, neat, and legible daily logs in a timely manner. 

Daily logs will be forwarded to The Company s Safety Department for audit and review. 

Once drivers have completed their daily logs, they are required to submit the original directly or forward it by mail after completionof a work assignment or within X days of completion if possible, but no later than 13 days (as required by regulation). 

I can see by your efforts that you take a great deal of pride in your work, and your efforts have not gone unnoticed. Filling out and submitting daily logs is an important and difficult part of your job. 

Truck drivers must complete daily logs and submit them to their employers, who keep them under rales of the Federal Motor Carrier Safety Office.
