In this chapter we are going to explore some of the more experimental uses of saw palmetto, looking at cross-cultural analysis of the herb as well as possible medicinal applications based on theoretical data or limited clinical experience.

In some diseases a simple ordinal scale or a VAS scale cannot describe the full spectrum of the disease. There are many examples of this including depression and erectile dysfunction. Measurement in such circumstances involves the use of multiple ordinal rating scales, often termed items. A patient is scored on each item and the summation of the scores on the individual items represents an overall assessment of the severity of the patient s disease status at the time of measurement. Considerable amoimts of work have to be done to ensure the vahdity of these complex scales, including investigations of their reprodu-cibihty and sensitivity to measuring treatment effects. It may also be important in international trials to assess to what extent there is cross-cultural imiformity in the use and imderstand-ing of the scales. Complex statistical techniques such as principal components analysis and factor analysis are used as part of this process and one of the issues that need to be addressed is whether the individual items should be given equal weighting.

The FAQLQ-PF and -TF were developed and validated in four stages (1) item generation using focus groups with both children and parents, expert opinion, and literature review (2) item reduction, using clinical impact and factor analysis (3) internal and test-retest reliability and construct validity were evaluated and (4) cross-cultural and content validity was examined by administering the questionnaire in a US sample (FAQLQ-PF, only).

This inquiry will require thick description and careful analysis of language in real situations and in experimental studies that test these results across many different sites of rhetorical production. In my own work, I have begun to explore the implications of this work in training in one particularly diffi-cult cross-cultural situation in the new South Africa. 1 hope that others will join me in this project. 

In addition, surveys from subjects who did not complete the survey both times (pre- and post-surveys) were kept for the analysis. In cross-cultural samples, each individual is not necessarily indq)endent from the rest of the population hence deleting the case could introduce bias to our estimations and errors (Dow and Eff 2009).
