Hydrocortisone and Prednisolone. Historically the only melanocortin peptide to be used clinically is the parent hormone from which all these peptides are derived from namely ACTH (see above). It has also been used in the treatment infantile spasms for epilepsy, where it is administered as an intramuscular injection only over a 2-12 weeks period. Obvious side effects include weight gain, puffy face, high blood pressure and an increased risk of infection and should never be administered to patients with diabetics, renal or heart failure. ACTH is also used as a stimulation test to measure adrenal cortex activity, i.e. production of cortisol and is used to ascertain whether someone has Addison s disease. [Pg.753]

Interference with corticosteroid function and the stress response has been shown for a variety of chemicals, including the pharmaceutical salicylate (Gravel and Vijayan 2006) and the PAH, phenanthrene (Monteiro et al. 2000a, 2000b). Other classes of chemicals shown to have significant effects on cortisol levels include PCBs and PAHs (Hontela et al. 1992,1997). Cortisol is an important component of the body s response to physical and psychological stress. Nervous signals regarding stress are transmitted to the hypothalamus and the release of CRH is stimulated. The resulting increase in cortisol increases levels of glucose, free fatty acids, and amino acids in the blood, providing the metabolic fuels that enable the individual to cope with the stress. A potent inhibitor of this system is cortisol itself. This hormone exerts a negative-feedback effect on the hypothalamus and the adenohypophysis and inhibits the secretion of CRH and ACTH, respectively. [Pg.135]
