As referred to above, most of the modules involve the intervention of a third party, the so-called notified bodies. These bodies are designated by Member States as being competent to carry out the certification tasks for which they are notified. As a precondition for their notification they must fulfil the minimum criteria which are set out in the relevant directives. These conditions vary from independence, impartiality and professional integrity, to technical competence, possession of or access to necessary facilities and to liability insurance. Any designated, trained (8-hour HAZWOPER supervisor course as a minimum), and experienced individual responsible for the safety of an employee (such as team leaders or crew leaders) may perform the function of an experienced person to provide the supervised field experience required by HAZWOPER [1]. Although having the appropriate certificates of completion would satisfy regulatory requirements, you should also consider time of service and experience. A fresh college graduate with training certificates and minimal field experience may be less desirable to perform supervised field experience than the safety professional with years of substantial field experience.

The easiest way to get information about family medical history is to talk to relatives about their health. Have they had any medical problems, and when did they occur A family gathering could be a good time to discuss these issues. Additionally, obtaining medical records and other documents (such as obituaries and death certificates) can help complete a family medical history. It is important to keep this information up-to-date and to share it with a healthcare professional regularly. 

Surveys of professional lawn chemical applicators were conducted during the summer and fall of 2004. Subjects recruited included participants in Ohio State University s OARDC (Ohio Agricultural Research and Development Center)-sponsored turf care professional educational seminars (Northeast Ohio Lawn Care Seminars). If you are currently in the process of earning a certificate or professional license, this should also be listed within this section of your resume, along with the expected completion date.

To be credentialed, health care professionals usually are required to submit specific information to the insurance carrier, such as license numbers, malpractice insurance information, board certifications, and copies of diplomas. Health insurance plans may have specific requirements, such as requiring physicians and other health care professionals to have specific credentials, carry a specified amount of malpractice insurance, or be board certified. The health care professional typically submits this information along with an application (Snella, 1999).
