Docetaxel Carboplatin

Docetaxel 75 mg/m2 IV infused over 1 hours + carboplatin AUC = 5 IV infused over 1 hour. Regimen is given once every 21 days x six cycles.

Four to six cycles of doublet chemotherapy with cisplatin or carboplatin plus docetaxel, gemcitabine, paclitaxel, or vinorelbine are recommended as first-line chemotherapy for patients with unresectable stage III or IV NSCLC. No combination was found to be superior tolerance of expected toxicities may contribute to the decision. 

Chemotherapeutic agents that have significant cancer response when combined with hyperthermia (up to 43°C) include doxorubicin, melphalan, mitomycin C (MMC), mitoxantrone, gemcitabine, etoposide, and especially the platinum-based agents carboplatin and oxaliplatin (Mohamed et al., 2003 Sugarbaker et al., 2005). Agents that do not work well with hyperthermia include irinotecan, paclitaxel, docetaxel, 5-fluorouracil, and floxuridine (Mohamed et al., 2003 Sugarbaker et al., 2005).

DHA-paclitaxel (Taxoprexin ) 62 (Luitpold) is being evaluated in a Phase III trial for the treatment of advanced non-small cell lung cancer in combination with carboplatin,136 as well against a variety of cancers in Phase II trials. DHA-paclitaxel 62,137 139 which is the C-2 docosahexaenoic acid (DHA) ester of paclitaxel 60, was licensed by Luitpold from Protarga in 2003. Polyunsaturated fatty acids have been reported to be taken up by tumour cells at a higher rate than normal cells and, as a consequence, a polyunsaturated fatty ester such as DHA-paclitaxel 62 may be absorbed more specifically in tumour cells. In preclinical studies, DHA-paclitaxel 62 exhibited increased activity in mice xenograft models and was more stable in vivo than paclitaxel 60. However, DHA-paclitaxel 62 may still suffer from efflux problems associated with paclitaxel 60 and docetaxel resistant tumours.

A number of cytotoxic agents have demonstrated significant singleagent activity in chemotherapy-naive patients with limited- and extensive-disease SCLC, but the activity in recurrent or refractory SCLC is modest. Among the more commonly used chemotherapy agents in the United States are cisplatin, carboplatin, etoposide (intravenous and oral regimens), topotecan, paclitaxel, docetaxel, and gemcitabine.
