The patient should seek medical attention when symptoms are severe, last longer than 3 weeks, are disabling, when alarm symptoms occur, or whenever a significant change in usual bowel habits occurs. 

All laxatives are contraindicated in patients with abdominal pain, nausea, vomiting, symptoms of appendicitis or undiagnosed abdominal pain. Patients should consult their physicians if sudden changes in bowel habits persist for more than fourteen days or if use of a laxative for seven days results in no effect. 

Changes in bowel habits, anorexia, nausea and vomiting, weakness (if anemia is severe)... 

Less than one-half of patients develop symptoms of recurrence such as pain syndromes, changes in bowel habits, rectal or vaginal bleeding, pelvic masses, anorexia, and weight loss. CEA levels may help detect recurrences in asymptomatic patients.

IBS is an idiopathic chronic, relapsing disorder characterized by abdominal discomfort (pain, bloating, distention, or cramps) in association with alterations in bowel habits (diarrhea, constipation, or both). With episodes of abdominal pain or discomfort, patients note a change in the frequency or consistency of their bowel movements. 

Change in bowel habit Patients over the age of 45 should be further investigated if they present with an alteration in bowel habit that lasts for two weeks or more. Diarrhoea may be bloody and could be associated with a sense of incomplete defecation.

Change in bowel habits (generally an increase in frequency) or rectal bleeding. 

Colon cancer has few symptoms in the early stages, making screening important. Warning signs that may indicate colon cancer are blood in the stool, change in bowel habits (including formation of the stool), or abdominal pain. 

The indications for CTC closely follow the indications for conventional optical colonoscopy with few exceptions. These indications include screening asymptomatic high- and average-risk patient populations, pre-operative assessment of the colon proximal to an obstructing mass, evaluation of patients with change in bowel habits, surveillance of patients post colorectal cancer surgery, and incomplete or failed colonoscopy. Patients with bleeding diathesis, contraindications to sedation, and frail and elderly patients may also be better suited for CTC than conventional colonoscopy.

This preparation is routinely used at our institution in all patients referred for screening for colorectal cancer, change in bowel habit, iron deficiency anaemia, constipation, heme positive stool, etc. (see Chap. 2). Intravenous contrast is never used as the polyps may enhance and simulate tagged stool causing a false negative finding. 

The primary colonic lymphoma is usually found in middle-aged or elderly people. Males are twice as often affected as females. Common symptoms include abdominal pain, weight loss, and changing bowel habits with an average duration of about 4-6 months. Primary colonic lymphomas occur more frequently in the setting of inflammatory bowel disease and immunosuppression and are found most commonly in the cecum or the rectum (Breneton et al. 1983) (Fig. 13.17a-c). 

Placebo-controlled trials In a randomised, double-blind study, the efficacy of 16 weeks of administration of tetra-hydrobiopferin (BH4) at reducing the core symptoms of aufism specfrum disorder was evaluated in 46 children aged between 3 and /years old. The most common adverse events (BH4 vs placebo) included irritability (22% vs 17%), sleep difficulty (9% vs 17%), change in bowel habits (0% vs 17%), repetitive behaviour (4% vs 9%), hyperactivity (9% vs 4%), and transient viral rash (9% vs 0%). These events were described as mild, with no significant differences in between the BH4 and placebo groups [2(T]. 

Most patients with colon cancer are asymptomatic early but may develop changes in bowel or eating habits, fatigue, abdominal pain, and blood in stool.
