Best available treatment

Maximum Contaminant Level (MCL) - The highest level of a contaminant that is allowed in drinking water. MCLs are set as close to MCLGs as feasible using the best available treatment technology and taking cost into consideration. MCLs are enforceable standards. 

Direct discharges are discharges from point sources into surface water pursuant to a National Pollutant Discharge Elimination System (NPDES) permit. NPDES permits are granted on a case-by-case basis and limit the permissible concentration of toxic constituents or conventional pollutants in effluents discharged to a waterway. These limits are generally established on the basis of the best available treatment technology and, where necessary, to protect surface water quality standards. 

Many therapists use e-mail to interact with clients. The advantages to this process are that e-mail maximizes client access to the therapist while minimizing the intrusion of client contact into the therapist s personal time. Homework assignments can be discussed and clarified between sessions, and between-session questions answered. In addition, e-mail introduces the possibility that a client can access a therapist hundreds or even thousands of miles away. I am a firm believer that face-to-face therapy is ideal for many important reasons (e.g., developing the therapeutic alliance), but in those instances in which clients are isolated from good care, e-mail therapy can be a very attractive option. This option allows the possibility for equal access to the best available treatment and therapy resources even if those resources are not available to the client locally. In addition, as interactive telephones become more standard, therapy by phone may allow distance therapy to become more personalized, and this medium may ultimately supplant e-mail as a way to seek therapy from a distance. 

In the United States a comprehensive document has been pubhshed by the EPA [25] outlining Best Available Treatment options and the regulatory treatment of effluents arising from pharmaceutical manufacture. However, the most comprehensive control system is currently that provided in the EU by the Directive on Integrated Pollution Prevention and Control [26], and this is becoming a model for the development of similar legislation across the world. 

Injection of BoNT is currently recognized as the best available treatment for dystonias and for certain types of strabismus, and new uses are continuously found (Bhidayasiri and Truong 2005 Montecucco and Molgo 2005). In addition, BoNT/A inhibit ACh release at autonomic nerve terminals which innervate the glands and smooth muscle, and it is currently used to treat diseases such as hypersalivation and hypersudoration (Brisinda et al. 2004 Naumann and Jost 2004). 

Mrs GG will benefit by having the best available treatment to reduce her chances of suffering a fracture if she suffers a further fall. 

The healthcare team caring from Mrs GG may feel that they have failed her by her refusal to accept the best available treatment. 

Legislating to reduce the price of drugs and allowing insurance companies not to pay for the best available treatment is not going to help. You would have to have yet more patients treated for each disease. 

The design of randomized clinical trials introduces ethical issues.Usually, study designs prevent the treatment from being modified because of the need to collect sufficient data to allow valid statistical inference. Ethically, clinicians are required to provide their patients with the best available treatment however, the justification for a randomized clinical trial is simply that the best treatment is not yet known. 

Chemical analysis, however, continues to be used to assess the quality of industrial and municipal wastes, as well as to assess the limits of concentration or loading that can be applied to the environment. Such limits have been tied to best-treatment performance of waste treatment plants, also termed best available treatment (BAT) and best available treatment economically achievable (BATEA).
