Direct cardiac delivery into adults can be performed via the vector transfer procedure described by Ikeda et al. (2002). Briefly, animals are anesthetized with an intraperitoneal injection of sodium pentobarbital (75 mg/kg), and ventilated. The right carotid artery is then cannulated with a catheter placed at the aortic root. The animal is then placed under induced hypothermia until the body core temperature drops to below 26 °C. The ascending aorta and pulmonary arteries are occluded and histamine pre-treatment is delivered to the aorta (20 mmol/1, volume 2.5 ul/g, body weight) for 3 min. The vector solution is then injected, the occlusions released 30 s later and the animal is resuscitated.

Approximately 90% of atherothromboembolic strokes in whites are caused by atheroma in the extracranial vessels, whereas intracranial disease appears to be equally important in blacks and Hispanics (Sacco et al. 1995 Wityk et al. 1996). Atheromatous disease in the ascending aorta and the aortic arch is increasingly recognized as a source of cerebral emboli and an independent risk factor for ischemic stroke in vivo (Amarenco et al. 1994 Jones et al. 1995 Heinzlef et al. 1997 MacLeod et al. 2004). 

The right hemisphere was more affected in this case, because the contrast medium injected into the left ventricle or ascending aorta during angiography is likely to reach the right brachiocephalic artery first. The hyperdensity of the affected cerebral hemisphere seen on CT scanning was due to leakage of the iodinated contrast medium into the extracellular space, because of an increase in the permeability of the blood-brain barrier. 

A 48-year-old man with an inflammatory aneurysm of the ascending aorta and severe heart failure due to massive aortic regurgitation was given a continuous intravenous infusion of milrinone O.Smicrograms/kg/ minute (1). His pulmonary arterial pressure fell and his symptoms improved over 7 weeks while he was taking corticosteroids. The diseased tissue was successfully replaced at operation and the milrinone was tapered uneventfully. 

Perfuse the animal for IS min through the ascending aorta with either 4% paraformaldehyde (p-FA) in 100 mM phosphate buffer, pH 7.2 (for light microscopy) or with 1 % GA and I % p-FA in 100 mM sodium cacodylate buffer, pH 7.15 (for electron microscopy). Heat fixative to 40 C before use.

They have found intimal thickening of the aortic root including the area of the coronary arterial ostia. The thickening was produced by the deposition of fibrous tissue on the internal elastic membrane of the proximal ascending aorta. The degree of intimal proliferation varied from minimal to extremely severe.

Make a small incision at the bottom apex of the left ventricle. Insert a blunt 13 G hypodermic needle upward through the ventricle past the aortic valve so that it can be visuahzed at approx 5 mm inside the ascending aorta. Clamp the needle in place with a hemostat across the ventricle. 

As soon as deep anesthesia has been induced, open the thorax and insert the cannula (connected to the perfusion apparatus via the two-way tap) into the heart via the left ventricle. Push the cannula forward until its tip is visible m the ascending aorta...
