Accidents Summaries

In this section, two topics will be introduced general summary statistics which can be used for most types of numerical information and specialised accident summary statistics.

Traditionally, the design and development of corporate SHE information systems has been a concern for the SHE department in the company only. The SHE experts of the company have been the owners of the system. They have felt that they need better tools to store and retrieve accident reports and to generate periodic accident summaries. Usually, the introduction of computer support has been a key issue. 

Accidents involving fire have occurred ever since man began to use flammable liquids or gases as fuels. Summaries of such accidents are given by Davenport (1977), Strehlow and Baker (1976), Lees (1980), and Lenoir and Davenport (1993). The presence of flammable gases or liquids can result in a BLEVE or flash fire or, if sufficient fuel is available, a vapor cloud explosion. 

This report documents the development of data on the severity as well as the frequency of accidents involving truck, rail, and air transport. Volume 1 includes a summary giving the probability of occurrence of accidents as a function of accident severity. Subsequent Volumes give supporting data, calculations and analysis.

In summary, the H-acceptor pairs appear to be very similar to their silicon counterparts, which we have discussed in depth. The H-donor pairs are similar in that the H occupies a silicon-antibonding site however, this is an antibonding site to the defect and not to the host as is found in silicon. It is also interesting to note that the computed hydrogen frequencies appropriate to the latter pairs are better described by theory than the silicon counterparts discussed earlier. It is not clear whether this is a consequence of the electronic-structure method used here, a natural consequence of the differences between the silicon and compound-semiconductor hosts, or simply an accident. 

In addition to describing protection, the summary should point out any mitigation systems at the facility which were not included in the PrHA (Mitigation is not normally included in a PrHA. See Section 3.2). Mitigation systems have the potential to reduce accident impact levels. They include spray systems to reduce release quantities and early warning systems. 

Use the Flixborough Works accident and the investigation developed in Problem 12-1 to develop an investigation summary similar to Example 12-2. Include layered recommendations to cover the accident causes and underlying causes. 

Develop an investigation similar to Example 12-1 and an investigation summary for an automobile accident that occurred as a result of a brake failure. Create your own brief accident scenario for this problem. 

The cause of a specific accident frequently places it in more than one category. Each of these sections includes descriptions of several accidents and a summary of the lessons learned. 

In order to demonstrate the use of the analysis flow scheme of Figure 35, one of the accidents from the FACTS database is described and analysed according to this analysis flow scheme. The accident description in the FACTS database is very detailed and only a summary is given here.

Geriatric Considerations - Summary Chlorzoxazone is a centrally acting skeletal muscle relaxant that at higher doses can cause CNS depression. It appears to have modest clinical benefits and is most often used for acute muscle discomfort as compared to chronic muscle pain. It is often taken in combination with acetaminophen. Transient CNS depressant effects place the older adult at risk for falls and related accidents when taking this drug.
