Start bupropion-SR (Zyban or Wellbutrin SR ) 1-2 weeks before quit date. 

Currently, bupropion (Zyban, GlaxoSmithKline) is the only approved drug for helping cease cigarette smoking, and finasteride (Propecia, Merck) has been approved by the Food and Drug Administration (FDA) for the treatment of male baldness. 

Care should be taken when prescribing other medications with clozapine. The mood stabilizer carbamazepine (Tegretol) and perhaps the antidepressant mirtazap-ine (Remeron) should not be taken with clozapine because they might further increase the risk of agranulocytosis. Likewise, the antidepressant bupropion (Wellbutrin, Zyban) should not be taken with clozapine because it may add to the seizure risk. 

Bupropion (Weiibutrin, Zyban). Bupropion is an antidepressant that is also FDA approved as a treatment for smoking cessation. It is marketed as an antidepressant as Weiibutrin and for the treatment of nicotine dependence as Zyban. Its mechanism of action in this regard is obscure, but it may act on dopamine and/or norepinephrine systems. 

Atypical Antidepressants. None of the so-called atypical antidepressants have been tested in the treatment of AN. However, mianserin, an antidepressant available in Europe, has been found to increase body weight in patients with various depressive disorders. Although bupropion (Wellbutrin, Zyban) has not been tested in the treatment of AN, it is effective in the treatment of BN. However, immediate-release bupropion is associated with an especially high risk for seizures in these patients and is therefore contraindicated in those with eating disorders. The seizure risk associated with sustained-release bupropion remains unclear at this time, as the doses studied have not been as high as those for immediate-release bupropion. 

Bupropion (Wellbutrin, Zyban). Bupropion is a newer atypical antidepressant that was initially suggested to increase both norepinephrine and dopamine activity in the brain, though controversy surrounds this hypothesis. Although bupropion has not been studied extensively in ADHD, early evidence does indeed indicate that it may be effective for both inattention and hyperactivity/impulsivity. Its effectiveness for ADHD does not appear to rival the stimulant medications, though a recent controlled study for adult ADHD showed that bupropiou outperformed placebo.

In May 1997, GlaxoSmithKline (then known as Glaxo Wellcome, Inc.) received FDA approval to market a sustained-release version of the antidepressant Wellbutrin (bupropion hydrochloride) to smokers under the name Zyban . Today, Zyban has captured one quarter of the smoking cessation market.

Suicidality in children and adolescents Although Zyban is not indicated for treatment of depression, it contains the same active ingredient as the antidepressant bupropion medications Wellbutrin, Wellbutrin SR, and Wellbutrin XL Antidepressants increased the risk of suicidal thinking and behavior (suicidality) in short-term studies in children and adolescents with Major Depressive Disorder (MDD) and other psychiatric disorders. Anyone considering the use of bupropion or any other antidepressant in a child or adolescent must balance this risk with the clinical need. Closely observe patients who are started on therapy for clinical worsening, suicidality, or unusual changes in behavior. Advice families and caregivers of the need for close observation and communication with the prescriber. Bupropion is not approved for use in pediatric patients. 

Depression Bupropion (immediate release) IR, Wellbutrin (sustained release) SR, and bupropion (extended release) XL are indicated for the treatment of depression. Smoking cessation (Zyban only) Indicated as an aid to smoking-cessation treatment. 

Usual dosage for adults-The recommended and maximum dose of Zyban is 300 mg/day, given as 150 mg twice daily. Dosing should begin at 150 mg/day given every day for the first 3 days, followed by a dose increase for most patients to the recommended usual dose of 300 mg/day. There should be an interval of at least 8 hours between successive doses. Doses above 300 mg/day should not be used. Zyban should be swallowed whole, not crushed, divided, or chewed. 

Treatment with Zyban should be initiated while the patient is still smoking, because approximately 1 week of treatment is required to achieve steady-state blood levels of bupropion. Treatment with Zyban should be continued for 7 to 12 weeks. If a patient has not made significant progress towards abstinence by the seventh week of therapy with Zyban, it is unlikely that he or she will quit during that attempt, and treatment should probably be discontinued. Dose tapering of Zyban is not required when discontinuing treatment. 

Maintenance - Systematic evaluation of Zyban 300 mg/day for maintenance therapy demonstrated that treatment for up to 6 months was efficacious. Whether to continue treatment with Zyban for periods longer than 12 weeks for smoking cessation must be determined for individual patients. 

Combination treatment with Zyban and a nicotine transdermal system (NTS) -Combination treatment with Zyban and NTS may be prescribed for smoking cessation. 

Monitoring for treatment-emergent hypertension in patients treated with the combination of Zyban and NTS is recommended. 

Other bupropion medications Do not use bupropion in combination with Zyban or any other medications that contain bupropion.
