Although there are many attempts have been made to describe road accident problem, I used to describe the problem as a disease where the causes of road accidents are symptoms as (illness). The road safety problem is a sign of illness in society. To diagnose the problem, there are direct and indirect symptoms (causes). The direct symptoms in road safety can be easily seen in society from simple observation and data such as speed problems, alcohol and driving, vehicle conditions, road user behaviour, etc. The indirect symptoms in road safety are not simply obvious and they need more checking and examination such as the traffic management, education, traffic police enforcement, legislations, etc. 

Vehicle condition and maintenance records will be checked page-by-page to see if the vehicle had been maintained and repaired correctly. 

The experts will review everything (logs, trip paperwork, dispatch records, company and driver receipts, credit card and phone records, electronic records, vehicle condition, etc.) to verify compliance by the driver and carrier, and the operational feasibility of the trip. What they are looking for is any place the carrier is not in compliance or below industry standards. If the attorney can prove that the carrier was not in compliance or was well below industry standards, the carrier has not followed its duty to act. This can lead to the carrier being potentially exposed to a large litigation award. 

The vehicle was current on all service requirements, clean, and the driver had no complaints as to condition. The mirrors were found to be adjusted too far in, narrowing the driver s visibility. The truck had been hit by another truck six months previous, requiring the right mirrors be replaced (vehicle condition and history). 

As well as locating and preserving documentation on the vehicle condition, locate and preserve any documentation pertaining to the involved driver s and vehicle s activities and movements over the last 30 days (minimum, check with your attorney) and every detail about the trip the driver was on at the time of the crash. The driver s qualifications and personnel file, and the vehicle maintenance file should be captured as they are the night of the accident. 

Not operate the vehicle if it would be unsafe (weather, vehicle condition, etc.). 

Vehicle conditions could be easily monitored by the use of in situ fabric-based sensors, which could detect damaged zones and the amount of damage. These e-textiles could help to reduce maintenance costs, minimise downtime and avoid undue personal and material loss on the battlefield.

As the vehicle is approached for inspection, follow all safety precautions. Do a complete walk-around of the vehicle and check for placards, leaks and general vehicle condition. 

These figures show that the highest level of bus accidents as a percentage of total road accidents was in Tanzania, and the highest level of fatalities per bus accident was in Nepal. Three main classifications of causes for bus accidents in Tanzania are shown in Figure 10.3 [20,28]. These classifications are human factors, vehicle condition, and external factors. The percentage breakdowns for these three classifications were 76%, 17%, and 7%, respectively. Clearly, the main cause of bus accidents in Tanzania was human error.
