At approximately 3 00 AM on May 9,2000 notification was made by the DCD EOC to the Utah Department of Environmental Quality (DEQ) and at approximately 3 34 AM to the Tooele County dispatcher. The event was classified as a Limited Area Event (not likely to leave the site). No action was taken by the state or county until normal business hours on May 9. 

November 18,1997— TOCDF is cited for 25 violations by the Utah Department of Environmental Quality. Citations included "numerous instances of noncompliance," but not enough to shut them down, according to a DEQ spokesperson.

The TOCDF QRA estimates the risk to the public and workers from accidental releases of chemical agent associated with all activities during storage at DCD and throughout the disposal process at the TOCDF. The HRA, which was conducted by the Utah Division of Solid and Hazardous Waste (Department of Environmental Quality), is a screening analysis to estimate possible off-site human health risks associated with exposure to airborne emissions from the TOCDF under normal and upset conditions. The HRA also estimates risks to wildlife and the environment.

Dr. Joe Comp retied from SUSC in 1989, Dr. Richman left SUSC in 1990 and Ty Redd took over management of the Water Lab. He quickly recruited several top freshman and sophomore chemistry students who demonstrated a diverse and desired set of personalities, such as attention to detail, reliability, initiative, and collegiality. These students were trained in the QA/QC practices of an environmental water laboratoiy. Dr. Redd formalized a model of peer mentorship by training a sophomore student to serve as the laboratory QA officer. This QA officer trained each new student worker, who was subsequently certified by the State Health Department on specific analyses. During this time period, good laboratory practices were fostered and maintained. The laboratory developed into a quality operation that provided consistent results, customer service, and retained Utah state certification. This certification required the lab to analyze a sample, obtained from the EPA, and get values within a certain threashold.
