The African Region is the WHO Region with the highest burden of tropical diseases. It is estimated that 80-90% of total clinical malaria cases (300-500 million) and malaria-related deaths (1.5-2.7 million) occur in Africa and about 55 million people are at risk of human African trypanosomiasis. Plague remains a public health problem in several countries, with around 1000 cases and 100 deaths yearly, and several countries in Central and Eastern Africa experience a high prevalence of onchocerciasis. Schistosomiasis and relapsing fever also add to the burden of tropical diseases, although they are endemic in more limited areas. [Pg.5]

Activities to control the vectors of schistosomiasis and human African trypanosomiasis remain limited but will be intensified in the future, particularly for tsetse fly control, for which insecticide-treated traps and screens represent the main method. [Pg.6]

Encephalopathic syndromes complicating treatment of stage II human African trypanosomiasis with melarsoprol in 588 patients have been reviewed (8). The overall rate of encephalopathy was 5.8% and presented in three ways coma, convulsions, and psychotic reactions. The overall death rate was 38%. Comatose patients had a death rate of 52% and were commonly co-infected with malaria (14/16). Symptoms during treatment of fever (RR = 11.5), headache (RR = 2.5), bullous eruptions (RR = 4.5), and systolic hypotension (RR = 2.6) were associated with an increased risk of encephalopathic syndromes, especially coma. [Pg.2244]
