When the project team assembles, the first order of business should be to put in writing the project objectives. Why are we doing this project This should be stated in the most basic terms. 

One important step to increase confidence in the monitoring process will be to ensure that representatives of principals (e.g., local stakeholder groups) are included in the teams assembled to investigate any serious chemical events. In addition, each site should develop clear and specific protocols that reflect the need to quickly, openly, and thoroughly inform all relevant principals of chemical events.

At some point near the end of the new construction period, usually lasting five or so years, every submarine obtains its initial SUBSAFE certification. This process is very formal and preceded by scrutiny and audit conducted by the shipbuilder, the supervising authority, and finally, by a NAVSEA Certification Audit Team assembled and led by the Office of Safety and Quality Assurance at NAVSEA. After all, if lethal medical accidents can occur in our best institutions, accidents can happen anywhere, and no organization is immune.

Table 9.1 is a matrix of the major safety analysis techniques described in previous chapters. Use it as an easy reference to get a feel for the different types of analyses. However, it is very limited and should be used only to get a notion of how the tools stack up with each other. For example, the costs of these techniques are highly dependent on the complexity of the system. Also, the time required to complete the analysis is also contingent on the size and complexity of the process. You should be very careful not to accept the time to perform the analysis too literally. It is necessary to give some sort of idea, but it is only that—an idea— not an absolute time frame. It is highly dependent on the process, how mature the system is, the team assembled, how experienced the crew is, and how many problems there may actually be. 

To validate and demonstrate the applicabihty of the template to communities of different sizes in different regions of the country, the BWIRP team assembled and conducted investigational workshops with local first responders and emergency-management teams in three communities Wichita, Kansas Pinellas County, Florida and Dover, Delaware. In all three, the template proved to be a valuable starting point for the development of customized emergency response plans.
