Initial Certification

Initial certification is separated into four elements (figure 14.3)  

Fabrication certification Once the certified material is obtained, the next step is fabrication where industrial processes such as machining, welding, and assembly are used to construct components, systems, and ships. OQE is used to document the industrial processes. Separately, and prior to actual fabrication of the final product, the facility performing the work is certified in the industrial processes necessary to perform the work. An example is a specific 

Each of these certification elements is defined by detailed, documented SUBSAFE requirements. 

At some point near the end of the new construction period, usually lasting five or so years, every submarine obtains its initial SUBSAFE certification. This process is very formal and preceded by scrutiny and audit conducted by the shipbuilder, the supervising authority, and finally, by a NAVSEA Certification Audit Team assembled and led by the Office of Safety and Quality Assurance at NAVSEA. The initial certification is in the end granted at the flag officer level.

Brief sterilizer equipment or process description shall be included for initial certification. 

Financial obstacles during the conversion period were relevant for the organic farmers. Relatively high costs of initial certification and the annual inspection, which are dependent on the certification body and also partly on the size of the farm, are also seen as problematic. These costs can often not be recouped, especially during the conversion period, as farmers frequently lack possibilities... 

Actual MTBF experienced will probably differ from predicted MTBFs used in FTA calculations during initial certification. Direct advantages are possible if the FTA can be updated with actual MTBF data either action can be taken to improve safety or (if the predicted MTBFs were very conservative) maintenance intervals can be increased. 

An Electrical Installation certificate should be used for the initial certification of a new electrical installation or for an alteration or addition to an existing installation. 

The key reasons driving the development of these technical programs are (1) rapid advances in technology, (2) desire to eliminate accidents in the workplace, (3) potential catastrophic risks, and (4) new regulations and guidelines from the government. The Occupational Safety and Health Administration (OSHA) recently enacted a process safety management (PSM) standard that requires employers to train their employees on process fundamentals. This standard applies to initial certification and recertification of employees. (See Chapter 2 for more details on the PSM standard.)... 

In addition to meeting all training requirements for initial certification and for recertification, the restart training shall address the following ... 

The TAM describes the purpose and composition of the oral component of the examination process. This component is used when it is desirable for the proctor to have flexibility in probing problem areas and to allow the student to express responses and concepts in his own words. The oral examination may be performed as part of simulator exercises, in-plant drills, or as an independent examination. Two-day oral and operating examinations are required as part of initial certification, and 1-day oral and operating examinations are required for recertification. In addition, after successful completion of the Peer Evaluation, an operator is required to pass a collegiate-style oral board chaired by the Plant Manager. Likewise, supervisors are required to complete an oral board chaired by the Site President.
