Safety management system plans

The government plan for developing, communicating, and monitoring the system safety effort may be called the system safety management plan.

Prepare the Systems Engineering Management Plan (SEMP), the System Safety Management Plan (SSMP), and tailor the Quality, Configuration, Risk, and Information Management Plans to meet the needs of the project. 

Besides setting the culture through their own behavior, managers need to establish the organizational safety policy and create a safety control structure with appropriate responsibilities, accountability and authority, safety controls, and feedback channels. Management must also establish a safety management plan and ensure that a safety information system and continual learning and improvement processes are in place and effective. 

During program and project planning, a system safety plan, standards, and project development safety control structure need to be designed including policies, procedures, the safety management and control structure, and communication channels. More about safety management plans can be found in chapters 12 and 13. 

All of the information in the safety information system will probably not be in one document, but there should be a central location containing pointers to where all the information can be found. Chapter 12 contains a list of what should be in an operations safety management plan. The overall safety management plan will contain similar information with some additions for development. 

System Safety 2000 by Joe Stephenson. This book begins with a history of and the fundamentals of system safety. Then, the author moves into system safety program planning and management, along with system safety analysis techniques. About half of the book is devoted to those techniques. A safety generalist would find it a good and not too difficult read.

System safety management An element of management that defines the system safety program requirements and ensures the planning, implementation and accomplishment of system safety tasks and activities consistent with the overall program requirements (MIL-STD-882). 

System safety program plan A description of the planned methods to be used by the contractor to implement the tailored requirements of this standard, including organizational responsibilities, resources, methods of accomplishment, milestones, depth of effort, and integration with other program engineering and management activities and related systems (MIL-STD-882). 

During the concept phase, historical data and technical forecasts are developed as a base for a system hazard analysis. A Preliminary Hazard Analysis (PHA) is conducted during this phase. At the gross level, a Risk Analysis (RA) is performed to ascertain the need for hazard control and to develop system-safety criteria. Safety management will be doing the initial work on the System Safety Program Plan (SSPP). Three basic questions must be answered by the time the concept phase is completed ... 

The audit identifies weaknesses in the health and safety management plan or system. Little is achieved unless a procedure is estabhshed to ensure prompt follow-up on deficiencies. This procedure should include provision for target dates for remedial action and checks to confirm completion.
