Long-termtherapy may cause muscle wasting (especially in the arms and legs), osteoporosis, spontaneous fractures, cataracts, glaucoma, peptic ulcer disease, and CHE... [Pg.345]

The clinical picture is characterized by xanthomas, particularly on the tendons (especially the Achilles tendon). Xanthomas consist of cholesterol and cholestanol. The enzyme defect also results in disturbed vitamin D metabolism. Osteoporosis is thus observed quite often, with a tendency towards spontaneous fractures. (208) Striking clinical features are cerebral functional disorders (from deviant behaviour to severe dementia, motor disturbances and convulsions) as well as peripheral neurological symptoms caused by cholestanol deposits. (211) High concentrations of apolipoprotein B and cholestanol are found in the CSF. (213) Treatment is based on the administration of chenodeoxycholic acid (750 mg/day). Effectiveness is improved if HMG-CoA reductase inhibitors (e. g. pravastatin) are used concomitantly. (207, 212)... [Pg.599]

The spatial variation of crosslinks at bone-forming trabecular surfaces (within 50 gm) in patients with osteoporotic or multiple spontaneous fractures was significantly different from that in normal bone, and the collagen crosslink ratio was higher. It has been hypothesized that the matrix produced in osteoporosis might mature more quickly or undergo post-translational modifications for a longer time than normal bone matrix. [Pg.161]

With advanced age, there is an increased impairment of renal function which has been shown to correlate with elevated PTH levels and radiologic bone disease (11). When the impairment of function reaches the renal failure stage, 2HPT has been found in 80-94X of patients (M,60). While some patients are asymptomatic, osteodystrophy is often manifested as spontaneous fractures (femoral neck, vertebrae) or bone and joint pains (59). Chronic hemodialysis may cause an exacerbation of the 2HPT and increase the risk of bone disease (61,62). [Pg.251]
