Self-evaluation managing

Gathering audit data can be accomplished through observations, documents, and interviews. The data obtained is used to verify and validate that the process safety management systems are implemented and functioning as designed. Data gathering can be aided oy the use of audit samples, where a representative number of items are audited to draw a conclusion, and by using self-evaluation questionnaires.

As for the external quality systems, we deliberately avoided the description of official quality systems, such as the Joint Commission on Accreditation of Healthcare Organizations (accreditation). International Standariza-tion Organization (certification), and European Federation for Quality Management (self-evaluation), because it would have been impossible to cover them all. Instead, we abstracted the topic of quality, and all these external quality systems will probably assume most of the things discussed in this article. 

Continuous Improvement Through commitment, self-evaluation, and the flexibility to change, improvements in risk management results and efficiency should be sought continuously.

Questionnaires can be used by a facility to conduct a self-assessment or can be administered by an outside evaluator, by phone or in person. In any case the questionnaire typically focuses on identifying PSM activities and program content based on the requirements of the PSM model selected, and assessing condensed criteria for the effectiveness of management systems (as shown in Figure 4-2, page 78). 

Self-report questionnaires containing a mixture of open and forced-choice questions. These could be distributed amongst mentors, mentees, relationship supervisors, line managers and program co-ordinators. In addition, you could also use this method to consult with peers, customers and/or suppliers -i.e. anyone who could provide you with valuable evaluation data. Make sure, however, that you customize the questions to the group of respondents you are addressing.
