Saw palmetto extract

Saw palmetto extract can inhibit the enzyme 5-a-reductase in vitro. This enzyme converts testosterone into dihydrotestosterone (DHT), which in turn contributes to prostatic enlargement. Saw palmetto also appears to have an antiinflammatory effect and can reduce DHT binding to prostatic androgen receptors (antiandrogenic effect). Despite its proposed 5-a-reductase mechanism. [Pg.793]

Numerous (but not all) trials have indicated improvement in BPH symptom scores compared to placebo with 1 to 3 months of therapy. Saw palmetto extract appears to be equally effective as finasteride (see Chapter 63) but is less effective than aj-adrenoceptor antagonists. No information appears to be available on the use of saw palmetto in the prevention of hair loss. [Pg.793]

Saw palmetto extract is a fairly well tolerated, safe alternative to finasteride for long-term treatment of BPH, although tti-adrenoceptor blocking agents undoubtedly afford more rapid symptom relief. [Pg.793]

Cytochrome P450 2D6 and 3A4 activities. Saw palmetto extract, administered to healthy volunteers (six men and six women) for 14 days at generally recommended doses, did not alter the disposition of coadministered dextromethorphan and alprazolam primarily dependent on the CYP2D6 or CYP3A4 pathways for elimination b Cytotoxic activity. Saw palmetto is a palm that grows in the southern United States, and extracts from the fruit of this palm are used primarily to treat benign prostatic hypertrophy (BPH).27 Evidence suggests that saw palmetto extracts can reduce urinary restriction and increase... [Pg.610]

Medicinal herbs, or phytotherapy, have been used extensively for benign prostatic hyperplasia in Europe and are being used more commonly in the U.S. Sales of herbal medicine reached 4 billion in the U.S. in 1998, and sales of saw palmetto extract for treatment of symptoms attributable to benign prostatic hyperplasia exceeded 20 million, making it the seventh most commonly purchased medicinal herbal preparation. [Pg.513]

A systematic review and meta-analysis of randomized trials of S. repens in men with benign prostatic hyperplasia showed that saw palmetto extracts improve urinary symptoms and flow measures to a greater extent than placebo, and similar improvements in urinary symptoms and flow measures to the 5-alpha-reductase inhibitor finasteride with fewer adverse effects (6). [Pg.336]

Anti-inflammatory effects of saw palmetto also have been hypothesized to improve BPH symptoms (6). An acidic, lipophilic saw palmetto extract (Talso ) was shown in vitro to inhibit both the cyclooxygenase and 5-lipoxygenase pathways, preventing the formation of inflammatory-producing prostaglandins and leukotrienes (12). Finally, saw palmetto has been purported to stimulate immune function (13). [Pg.167]

Saw palmetto extract in a dose of 160 mg or placebo three times daily was administered to 35 elderly men, and prostatic tissue was collected (14). The investigators found that some component of the saw palmetto extract inhibits nuclear estrogen receptors in the prostates of patients with BPH patients. [Pg.167]

Potential problems There is concern that saw palmetto might hide elevated prostate specific antigen (PSA) levels, thus preventing early detection of prostatic cancer. Recent studies from France, published in The Prostate in 1996, show no change in PSA levels even after men have taken saw palmetto for one year. According to studies done by Italian phytopharmaceutical manufacturer Indena S. P. A., saw palmetto extracts are devoid of any mutagenic or teratogenic effects. [Pg.23]

Duvia et al. "Advances in the Phytotherapy of Prostatic Hypertrophy." The effectiveness and tolerability of saw palmetto extract was compared with pygeum (Prunus ajricanium) extract in 30 patients with prostatic adenomas. The saw palmetto was superior to the pygeum in reducing symptoms, and it was much better tolerated. The Serenoa did not produce any side effects, while the pygeum caused gastric upset in 13 percent of the patients. [Pg.69]

A recent study by Bach, Schmitt, and Ebeling compared three prescriptive medications with a standardized saw palmetto extract. The drugs worked by two different mechanisms. Finasteride is a synthetic 5-alpha-reductase inhibitor, and the other two drugs, terazosin and alfuzosin, are alpha-1 blockers. In all important parameters, the saw palmetto was a comparable treatment with far fewer side effects. It decreased residual urine volume, reduced irritation, and increased urinary flow 80 percent of patients rated their improved quality of life as good to very good. [Pg.71]

Saw palmetto s benefits in treatment of BPH are hypothesized to be due in part to antiandrogen effects (Wilt et al., 1998). Saw palmetto is a multisite inhibitor of androgen action. A mean peak plasma drug concentration of 2.6 mg/L of the "second component" with a high-performance liquid chromatography (HPLC) retention time of 26.4 min was measured in 12 healthy young men after a single oral dose of 320 mg of saw palmetto. The time to peak concentration occurred 1.5 h after administration (De Bemardi Di Valserra et al., 1994). A 640-mg rectal dose of saw palmetto extract produced a peak of 2.6 pg/rnL occurring 3 h after the dose (De Bemardi Di Valserra and Tripodi, 1994). [Pg.197]
