However, many of those in leadership positions take responsibility without being held accountable. Such a culture can discourage recognition of safe behavior and prevent criticism of unsafe or suspicious conduct by superiors or even peers. Hierarchical management structures thus can inhibit reliance on coworkers to report or prevent breaches in safety or security. 

What environmental factors are supporting a particular at-risk behavior and/or inhibiting, perhaps demotivating, a particular safe behavior ... 

If some employees are not sure of the safe way to perform a certain job, behavioral direction is needed. A global "percent safe score" is not sufficient. When the CBC is reviewed during a one-on-one coaching session, behavioral direction is provided. The worker sees what critical behaviors were observed as "safe" and "at-risk." A constructive conversation with the coach provides support for safe behavior and corrective feedback for behavior that could be safer. Often this includes suggestions for making the safe behavior more convenient, comfortable, and easier to remember. It might also include tire removal of barriers (physical and social) that inhibit safe behavior. 

Hauke (1985) was able to extract nine safety factors from existing literature and from material available in companies (i.e. accident reports), among them factors such as the quality of safety work, load and overload of job incumbents, inhibition of safe behavior (through bad tools, time pressure and so on). Although these factors can have an impact on safe behavior, it is not quite clear what type of intervention should follow the identification of such conditions. We believe that a safety factor should be clearly related to the kind of intervention which is implemented. Guided by these criteria we came up with three main categories for intervention ...
