Comparative studies Quetiapine 375 mg/day and risperidone 2.72 mg/day have been compared in 72 patients with a first episode of a schizophreniform psychosis with less than 2 weeks of exposure to antipsychotic drugs in a single-blind, 12-week, randomized, controlled trial [120 ]. Both drugs reduced immediate symptoms and there were relatively few adverse reactions other than weight gain. The median time to cessation for patients randomized to quetiapine was 65 days and for risperidone 83 days, and this difference was not significantly different. 

Comparative studies The SCoP study was a prospective, randomized, partially blinded, active-controlled, multinational trial, designed to assess the safety of sertindole in the treatment of schizophrenia under normal conditions of use risperidone was the comparator. Results are now emerging. For instance, sertindole did not increase allcause mortality, but cardiac mortality was higher and suicide attempts possibly lower with sertindole [147. The study comprised 9858 selected patients with schizophrenia, and after 14147 person-years the major findings were (i) no statistically significant difference between sertindole ( = 4905) and risperidone (n = 4904) in the two co-primary outcomes, i.e. all-cause mortality (sertindole 64 deaths, risperidone 61 deaths HR=1.12 90% Cl = 0.83, 1.5) and cardiac events...

Controlled trials comparing risperidone with typical drugs have provided inconclusive results overall. Of five non-randomized studies (Table 2.5) only one (Malla, 1998) indicated that risperidone use might result in lower costs by reducing time spent in hospital.

Raman often is evaluated as an alternative to an existing high performance liquid chromatography (HPLC) method because of its potential to be noninvasive, fast, simple to perform, and solvent-free. Raman was compared to HPLC for the determination of ticlopidine-hydrochloride (TCL) [43], risperidone [44] in film-coated tablets, and medroxyprogesterone acetate (MPA) in 150-mg/mL suspensions (DepoProvera, Pfizer) [45] it was found to have numerous advantages and performance suitable to replace HPLC. In an off-line laboratory study, the relative standard deviation of the measurement of the composition of powder mixtures of two sulfonamides, sulfathiazole and sulfanilamide, was reduced from 10-20% to less than 4% by employing a reusable, easily prepared rotating sample cell [46]. 

Of the atypical antipsychotics, clozapine, olanzapine, and risperidone have been studied the most. Clozapine was used to treat 10 treatment refractory acutely manic patients and 15 schizomanic patients. Using reduction in the YMRS score as the outcome measure, 72% improved (non-rapid cycling, bipolar patients). Comparison of olanzapine (5-20 mg) with placebo showed significant reduction of the YMRS in 49% vs. 24% of subjects by 3 weeks, with significant change evident by the first week. In a trial comparing risperidone at 6 mg with haloperidol at 10 mg and low-dose lithium (800-1200 mg/day) efficacy was similar over the 28 days of the trial. 

Studies of the use of the specific serotonin-uptake inhibitors (SSRIs) to treat OCD suggest that, compared to non-tic-related OCD, tic-related OCD is less responsive to SSRI monotherapy (McDougle et al., 1993, 1994). Addition of a neuroleptic, such as haloperidol (McDougle et ah, 1994), risperidone (McDougle et al., 2000), or olanzapine (Bogetto et al., 2000), appears to be useful in improving treatment-resistant individuals response to a SSRI. It is unclear whether this pattern of treatment response is specifically associated with a comorbid tic disorder the pattern of obsessive compulsive symptoms characteristic of TS or yet some other predictors.

In the Expert Consensus survey, the respondents endorsed risperidone, olanzapine, and quetiapine, in that order, followed by high-potency traditional antipsychotics, for managing self-injury. A placebo-controlled study comparing risperidone with a classical antipsychotic, such as haloperidol, could provide valuable data for this field.
