As stated more than once already, safety programs are often broken into pieces and the leadership, or ownership, of the parts is distributed. Yet, someone, and they know who they are, is ultimately responsible for the safety of all of the company s employees. These are the leaders who state, Safety is our no. 1 priority and Our employees are our most valuable assets. To have a world-class safety program, saying it is no longer enough. [Pg.122]

The patient safety program must include the development of a continuous assessment process to prevent error occurrence. Event information from data and incident occurrence reports should be reviewed by the multidisciplinary team to prioritize organizational efforts. Sources of data could include incident reports, medication errors, adverse drug reactions, transfusion reactions, sentinel events, and other adverse events. The patient safety program encompasses the patient population, visitors, volunteers, physicians, and staff and addresses improvement issues in every department. Senior leadership must be responsible for ensuring full implementation of the program, with an emphasis on the following functions ... [Pg.452]

The Bureau of Mines drew a different lesson from the disaster. Louis McCabe came back to Washington in mid-1949 to head its air and water pollution programs, and under his leadership the Bureau called for a federal air pollution program that went beyond research to control. Like the mine safety expert Daniel Harrington, who had just retired, McCabe was careful to avoid open disputes. However, it is probably fair to say that most professionals in the oflshore oil and gas industry accept that the management of catastrophic events has improved, but not nearly as dramatically as it has for occupational safety. The reason for this cautious response may be that an SMS program by itself is not enough—it is a necessary but not sufficient condition. To be effective, the SMS has to affect the culture of the oiganization, and the way people behave and act. The effectiveness of the program also depends on the leadership and decisiveness of the company management, as discussed in Chapter 8. [Pg.2]
