Ranitidine Itraconazole

Significant drug interactions with theophylline [268], itraconazole [269], warfarin, yS-acetyldigoxin, ranitidine, probenecid and oral contraceptives have not been reported with moxifloxacin [12,237]. 

Following the addition of itraconazole to a treatment regimen comprising warfarin, ranitidine, and terfena-dine, cardiac dysrhythmias developed in a 62-year-old man. The signs and symptoms included prolongation of the QT interval and ventricular fibrillation (SEDA-18, 198). 

Drug interactions Cimetidine is a CYP450 inhibitor and can inhibit the metabolism of phenytoin, warfarin, and theophylline. Famotidine, nizatidine, and ranitidine are unlikely to cause clinically significant drug interactions. All antagonists have the potential to interact with other drugs that require gastric acid for absorption (e.g., ketoconazole, itraconazole).

Clinically important, potentially hazardous interactions with aluminum, aminophylline, aspirin, chlorambucil, cimetidine, clarithromycin, cyclophosphamide, cyclosporine, dicumarol, diuretics, docetaxel, estrogens, grapefruit juice, indomethacin, influenza vaccines, itraconazole, ketoconazole, lansoprazole, live vaccines, methotrexate, montelukast, omeprazole, oral contraceptives, pancuronium, phenobarbital, phenytoin, ranitidine, rifampicin, rifampin, timolol, tolbutamide, vitamin A...

Eight healthy subjects were given itraconazole 100 mg with either 325 mL of water or Coca-Cola (pH 2.5). Their peak serum itraconazole levels were more than doubled by the Coca-Cola and the itraconazole AUC was increased by 80%. Two of the subjects did not show this effect. Another study in 18 fasted AIDS patients who absorbed itraconazole poorly, found that the absorption was restored to that of fasted healthy sub-jects when the itraconazole was given with a cola drink. A study in 30 healthy subj ects compared the bioavailability of itraconazole alone or after ranitidine, both with and without a cola drink. Ranitidine reduced the absorption of itraconazole but this effect was countered by the cola drink. ... 

The gastrointestinal absorption of ketoconazole is markedly reduced by cimetidine and ranitidine. The absorption of itraconazole is reduced (possibly halved) by H2-receptor antagonists, and, similarly, the absorption of posaconazole is reduced by about 40% by cimetidine. The absorption of fluconazole and voriconazole is not significantly affected by H2-receptor antagonists.
