Pathophysiologically, accelerated breakdown of fibrin occurs secondarily to disseminated intravascular coagulation or primarily in prostatic surgery. 

Erectile dysfunction, that is, the inability to maintain penile erection for the successful performance of sexual activity, has both organic and psychogenic causes, including as a sequelae to prostatic surgery. Erectile dysfunction is estimated to affect up to 30 million men in the United States. Previous therapies have included penile implants, and intrape-nile injections of alprostadil (see p. 420). Sildenafil [sil DEN a HI], the first oral drug approved for the treatment of erectile dysfunction in males, was introduced in early 1998. 

Societal changes also affect you. While the Pill would probably not be approved today, it sells well but more and more men and women want a pill for men. Viagra should not have been actively researched, but it found its indication of erectile dysfunction, common in diabetic men. Recently, some physicians have begun using Viagra and its competitors after prostate surgery with promising results. 

The principal indication for tranexamic acid is to prevent the hyperplasminaemic bleeding state that results from damage to certain tissues rich in plasminogen activator, e.g. after prostatic surgery, tonsillectomy, uterine cervical conisation, and... 

In men, SUI is most commonly the result of prior lower urinary tract surgery or injury, with resulting compromise of the sphincter mechanism within and external to the urethra. Radical prostatectomy for treatment of adenocarcinoma of the prostate is probably the most common setting in which surgical manipulation leads to UI. Overall, SUI in the male is uncommon, and in the absence of prior prostate surgery, severe trauma, or neurologic illness, is extraordinarily rare. Transurethral resection of the prostate for benign prostatic hyperplasia (see Chap. 82) may also lead to SUI in men.

What happened to Mr. Dole after he had prostate surgery is similar to what happened to President Franklin D. Roosevelt after he had polio. Each suffered from an interruption of nerve conduction, resulting in a condition called paralysis. Roosevelt did not have walking dysfunction he was paralyzed. Dole does not have sexual dysfunction he is impotent. The rhetoric he spouts is not medical science, it is junk science or pseudoscience, the same junk science that gives us behavioral dysfunctions, emotional dysfunctions, mood dysfunctions, and so forth. Each dysfunction, of course, is said to be treatable, as if calling a condition treatable proved that it is a disease. 

Trimethoprim sulfa (Bactrim), a sulfa antibiotic, to a client post-prostate surgery. 

There is no reason for the nurse to question Bactrim for a client who has had prostate surgery.

A hand-held multiarm concentric-tube robotic system for transurethral laser prostate surgery has been reported previously [49]. The system is expected to facilitate holmium laser enucleation of the prostate for treating benign prostatic hyperplasia. It consists of a camera, a 6-DOF arm for tissue manipulation, and a 3-DOF arm for laser guidance deployed through an 8.66-mm endoscope.
