Prospective observational studies with SBC-5-IMNs in 3 international centers indicated over 80-90% remission in early as well as late RA. These findings need to be supported by prospective double blind controlled trials. The RworalDs and RwD achieved can be maintained by immediate suppression of early flare. Even grade >2 erosions of joints can be healed when treated adequately.

Comaty et al. (452) conducted a prospective double-blind, placebo-controlled trial of low-dose benztropine (i.e., 2 mg/day) maintenance therapy. After an acute EPS had been stabilized with 2 days of active antiparkinsonian medication, the recurrence of EPS did not differ significantly between those maintained on active drug or those switched to placebo over the next 8 days. While there was a trend favoring the active drug group, low-dose maintenance benztropine (i.e., 2 mg/day) afforded little benefit in comparison with placebo.

There are two protein replacement products available for A1AD, both pooled human plasma donor-derived products. Approval of these products was based upon replacement of serum AAT to the above-mentioned levels. Neither has been proven to prevent lung disease in a prospective, double-blinded placebo-controlled fashion, but retrospective registry data suggest a beneficial effect. However, protein replacement therapy has been very safe. Particularly remarkable has been the lack of an adaptive immune response by patients on therapy, which may relate to the genetic homogeneity of this A1AD population, in which approximately 95% have one particular missense mutant allele (PI Z). 

One randomized control trial involving the use of abciximab during PPI demonstrated no long-term benefit in stent patency for complex superficial femoral lesions (35), However, a larger prospective, double-blind, placebo-controlled designed study that involved 98 patients showed that adjunctive administration of abciximab had a favorable effect on patency and clinical outcome in patients undergoing complex femoropopliteal catheter interventions not hampered by serious bleeding (36). 

In a prospective, double-blind, randomized, placebo-controlled study in 119 healthy volunteers (mean age 35 years), who took either atovaquone 250 mg/day + chlor-oguanide 100 mg/day or mefloquine 250 mg/week, depression, anger, and fatigue occurred during the use of mefloquine but not atovaquone + chloroguanide (510). Mefloquine can also cause psychosis. 

In a prospective, double-blind, placebo-controlled study, concurrent oral administration of 500 mg glutamic acid tds with vincristine reduced the incidence of subjective and objective signs of vincristine neurotoxicity (54). There were no differences in constipation, weakness, or loss of knee reflexes. There were no severe gastrointestinal adverse effects.

A single center, prospective, double-blind, placebo-controlled, crossover trial investigated the activity of Echinacea in humans for the treatment of recurrent genital herpes. The 1 -year study involved 50 patients who were each given the product Echinaforce for 6 months and placebo for 6 months. The study found no statistically significant benefit in using Echinaforce vs placebo for frequently recurrent genital herpes (15). 

Other studies have found that immunisation has no effect or even a protective effect on IgE-mediated disease. In contrast to the studies that found that the pertussis vaccine increased IgE-mediated disease, two studies formd that the pertussis vaccine had no effect on asthma and allergies. In a large prospective cohort study in the Avon health Authority, United Kingdom, there was no increased risk for wheezing illnesses in vaccinated children [172(1110]. In a randomised double-blind controlled trial of pertussis vaccine in Sweden, the crunulative incidence of IgE-mediated disease was similar in the three vaccination and placebo groups [164(Ib)]. 

By using multicenter, prospective, double blind, parallel, placebo controlled and randomized method, 50 patients with average age 67+11 (55-89) are administrated orally 0.2 mg (4 tablets) HupA and 53 patients with average age 66 + 11 (53-90) are given orally the same 4 tablets of placebo, bid for 8 weeks. The results indicated 58% (29/50) of patients treated with HupA showed improvement in their memory (P<0.01), cognitive (P<0.01) and behavioral (P<0.01) functions. The efficacy of HupA is better than placebo (36% 19/53) (P<0.05). No severe side effect was found. As a result, HupA is a promising drug for symptomatic treatment of Alzheimer s disease [38].

A prospective randomised double-blind controlled trial comparing lidocaine and chloroprocaine for outpatient transurethral prostatectomy yielded similar clinical end points (primary end point duration of spinal block) [36 ]. Four patients in the lidocaine group developed TNS and one patient in the chloroprocaine group developed an acute cauda equina syndrome, which fully recovered after several weeks. Based on this, the authors conclude that they cannot recommend lidocaine in view of the high incidence of TNS and recommend close follow-up for chloroprocaine to ensure its safety profile.
