The Center for Chemical Process Safety (CCPS) of the American Institute of Chemical Engineers (AIChE) has developed a Process Safety Incident Database (PSID), (http //www.psidnet.com), adapted from that of a major oil company, circa 1995. The database system is used to collect data from high learning value process safety incidents from participating... 

No one comprehensive data source contains the data needed to adequately understand root causes and lessons learned from reactive incidents or other process safety incidents.24 Table 1 lists the limitations of some public databases. 

PSCMS is primarily designed as a metric for tracking industry performance on process safety incidents it is not intended to be a lessons-learned database. However, if expanded to include causes and lessons learned and if more widely distributed, the data could be useful in preventing similar incidents. 

Industry databases containing details of process safety incidents are addressed in more detail in Chapter 11. 

Effective Materials and Spare Parts Description in Procurement Database. Improper stores or procurement descriptions allow inadequate parts, materials or components to enter the plant, increasing the potential for process safety incidents (if under specified) or cost overruns (if over specified). Personnel entering parts descriptions in electronic or paper ordering systems must be trained to ensure the accuracy of their work. See Chapter 2 under Database Discipline. 

It is a good practice to ensure that both the toller and client have records of the complete incident and accident history for the toll. This information can be vital to the industry as a database of experience with the tolled process. Process safety concerns for future tolls of the product or related materials can benefit from this practice. 

This chapter focuses on the value of critically analyzing incident information and discusses the benefits of using databases to evaluate lessons learned. The power of both internal databases and industry-wide incident databases can help organizations improve their performance in process safety and environmental responsibility. 

The Mary Kay O Connor Process Safety Center is conducting a number of projects on Development of Accident History Databases, Analyses of Accident History Databases, and Risk Assessment Based on Accident History Databases. One such project involves the compilation of incidents from various sources.
