Pinworm infection

Pyrantel Pamoate. This dmg (10), C24H2qN20 S, cures pinworm infections and is close to 100% effective against A.scaris when given in a single dose (28,29). Pyrantel pamoate is also a principal dmg for the treatment of hookworm infections thus it is useful in patients with mixed worm infections. 

When a pinworm infection is suspected, the nurse takes a specimen from the anal area, preferably early in the morning before the patient gets out of bed. Specimens are taken by swabbing the perianal area with a cellophane tape swab.

For intestinal capillariasis, mebendazole is used at a dosage of 400 mg/d in divided doses for 21 or more days. In trichinosis, limited reports suggest efficacy against adult worms in the intestinal tract and tissue larvae. Treatment is three times daily, with fatty foods, at 200-400 mg per dose for 3 days and then 400-500 mg per dose for 10 days. Corticosteroids should be coadministered for severe infections. 

Piperazine is also a drug of choice for treating pinworm infections in children and adults. A dose of 2-2.5 g of piperazine given daily for 14 days gives nearly 100% cures, while a dose of 50-75 mg/kg (maximum 0.5-3g) of piperazine citrate given for 7-14 days produces 85-100% elimination of . vermicularis in man with little or no toxicity [52-54,56,57]. Piperazine and its salts show poor activity against whipworms, Trichuris trichiura in man [63,64]. 

The most effective method of diagnosing pinworm infections is by the use of perianal swab using adhesive Scotch tape. The Scotch tape, which is applied to the perianal region with a tongue depressor, is examined microscopically for eggs. ...
