Maintenance hemodialysis has grown and expanded beyond the expectations of even the most enthusiastic of its eadiest proponents. Figure 7 is a plot of the overall estimated dialysis population by year siace 1970. The population at the end of 1992 exceeded 475,000 another 500,000 patients or so have received therapy at one time but have siace died or had transplants. Maintenance dialysis is now available to some extent ia all but the poorest nations ia economically advanced countries, excepting the United Kingdom, it is rendered as a virtual entitlement. The current worldwide mean cost of a single dialysis patient is about 30,000 per year (47) the aggregate economic magnitude of the medical appHcation of hemodialysis thus approaches 15 biUion. [Pg.37]

Generally, anticonvulsants reduce the excitability of the neurons (nerve cells) of the brain. When neuron excitability is decreased, seizures are theoretically reduced in intensity and frequency of occurrence or, in some instances, are virtually eliminated. For some patients, only partial control of the seizure disorder may be obtained with anticonvulsant drug therapy. [Pg.254]

Monitoring and Managing Adverse Drug Reactions Bleeding at virtually any site can occur during tiierapy with any heparin preparation, even the LMWHs. The nurse monitors the patient s vital signs every 2 to 4 hours or as ordered by the primary health care provider. [Pg.427]

Histopathological examination shows the typical corelike lesions in a high proportion of muscle fibers in older patients this may amount to 100%. Most typically the cores are large and centrally-placed, but multiple cores may occur in the same fiber cross section. Most older patients show a striking predominance of type 1 (slow twitch oxidative) fibers and virtually all fibers with cores are type 1. Sometimes younger family members have more normal proportions of type 1 and type 2 fibers but, again, the cores are confined to the type 1 fibers. It is well established that muscle fiber types can interconvert due to altered physiological demands, and it is likely that fibers with cores convert to a basically slow twitch-oxidative metabolism to compensate for the fact that up to 50% of their cross sectional area may be devoid of mitochondria. [Pg.292]

In severe neonatal nemaline myopathy virtually every muscle fiber shows multiple rods and all muscle fiber types are affected. However in juvenile cases, two different patterns of fiber type involvement are seen. In one there is a clear size difference between type 1 fibers, which are abnormally small (hypotrophic or atrophic) and which contain numerous nemaline rods, and type 2 fibers, which are either of normal diameter or hypertrophic and contain few, if any, nemaline rods. Other patients show a gross predominance of type 1 muscle fibers, again with rods virtually confined to this fiber type. These findings may be explicable in terms of the involvement of isoforms of a-actinin specific to slow and fast muscle fiber types. [Pg.294]

Vincristine a severe polyneuropathy is a particularly common side effect, but in some patients a painful autophagic myopathy is also present. Abnormal lysosomal complexes are also formed which function as virtually indigestible lipid accumulations. [Pg.344]

Interest in import of proteins into peroxisomes has been stimulated by studies on Zellweger syndrome. This condition is apparent at birth and is characterized by profound neurologic impairment, victims often dying within a year. The number of peroxisomes can vary from being almost normal to being virtually absent in some patients. Biochemical findings include an accumulation of very long chain fatty acids, abnormalities of... [Pg.503]

Normal saline is an isotonic fluid composed of water, sodium, and chloride. It provides primarily ECF replacement and can be used for virtually any cause of TBW depletion. Common uses of normal saline include perioperative fluid administration volume resuscitation of shock, hemorrhage, or burn patients fluid challenges in hypotensive or oliguric patients and hyponatremia. [Pg.405]

Hypothyroidism can affect virtually any tissue or organ in the body. However, the most common symptoms, such as fatigue, lethargy, sleepiness, cold intolerance, and dry skin, are nonspecific and are seen with many other disorders. The classic overt signs, such as myxedema and delayed deep tendon reflexes, are seen uncommonly now because more patients are... [Pg.667]

Following hematopoietic stem cell transplantation the patient will need virtually all routine vaccines to be administered again however, the patient will not be able to mount an adequate response for 6 to 12 months post-transplant. Diphtheria, tetanus, acellular pertussis, Haemophilus influenzae type b, hepatitis B, pneumococcal, and inactivated poliovirus should be given at 12,14, and 24 months post-hematopoietic stem cell transplantation. Inactivated influenza vaccine should be given yearly, starting 6 months after transplant. Measles, mumps and rubella can be given 2 years after transplant and varicella vaccine is contraindicated.16... [Pg.1249]
