The main expected advantage is that such an accelerator could be hospital-based, and thus fractionation (of the irradiation and drug administration) could be optimized since the machine would be available 24 hr a day. In addition, patient positioning could be made easier and more accurate. 

The other program involves systemic administration of the gene-laden adenovirus via a hepatic artery catheter to treat hepatocellular carcinoma (HCC). The hepatic artery supplies the normal liver with 25% of its blood supply. However, it is the sole blood supply for the carcinoma. Preclinical results demonstrated efficacy in a rat HCC model. In a small, open-label clinical trial of patients positive for HCC, but also having post-hepatitis cirrhosis, patient response was marginal [22].

In a single-arm, open, prospective study between 1990 and 1995 (before HAART) the prophylactic efficacy of Fansidar was evaluated in 95 HIV-infected patients with successfully treated Pneumocystis proved pneumonia and no history of Toxoplasma encephalitis (3). Patients took Fansidar with folinic acid (15 mg) twice weekly and were followed for a median of 19 (range 1-72) months. Five patients had a Pneumocystis relapse, but three had not taken their therapy. Of the 69 patients positive for. r. t. -Toxoplasma IgG antibodies, only one developed toxoplasma encephalitis after 50 months. A rash developed in 16 patients after a median of 3 weeks, and required withdrawal in six. Two developed Stevens-Johnson syndrome after three or four doses. There was no significantly increased risk of adverse reactions to Fansidar in patients with previous hypersensitivity reactions to co-tri-moxazole. The results of this study are of particular relevance to areas in which HAART is unavailable and where the antimalarial activity of Fansidar may confer additional benefit. 

The association between valproate and raised transaminase activities has been studied in a retrospective review of the medical records of patients positive for hepatitis C virus (83). Of 214 patients 28 were taking valproate and 186 were not. The controls and those who had been hepatitis-positive for longer were significantly more likely to have worse hepatotoxicity. Valproate, either alone or in the presence of other potential hepato-toxins, was not associated with raised transaminases.

The jugular venous pressure (J VP) is used as a measure of right atrial pressure. The JVP is measured in centimeters from the sternal angle and is best visualized with the patient s head rotated to the left. Dexamethasone suppression test An overnight test used to screen patients for Cushing syndrome by administering dexamethasone to a patient. Positive results for this test are indicated by a patient s inability to reduce cortisol levels after dexamethasone treatment—usually because the patient s feedback loop mechanism is ineffective at inhibiting cortisol release. 

Although PET/CT modality is widely employed for diagnostic purpose, CT data acquired either for fusion of PET and CT images or for photon attenuation correction are affected by several factors that cause artifacts in PET/CT images. The factors include patient positioning, metal objects, CT contrast agents, truncation artifacts, and respiratory movement, which are discussed below.

Selective screens (eg, for dmgs of abuse ) with rapid turnaround times are often used to confirm clinical impressions and may aid in disposition of the patient. Positive results should be subjected to confirmatory testing with a second method. 

The prototype system used in the trial was made by General Electric Medical Systems (Milwaukee, WI). The system used a 18 x 23 cm detector incorporating an amorphous silicon thin-film transistor bonded to a cesium iodide crystal scintillator. Other than the detector and associated electronics, the system was identical to one of the GE s clinical units, the DMR. For this reason, the GE DMR was chosen as the film unit for the trial. The digital and film units had almost identical dimensions, allowing the same patient positioning to be used on both. The only noticeable difference was the thickness of the digital detector on the prototype, compared with the film cassette used on the DMR.
