Paediatrics Circadian rhythms was assessed during prolonged midazolam infusion in 27 paediatric intensive care unit children under mechanical ventilation [13 ]. This study foxmd that endogenous rhythms in critically ill and sedated children are severely disturbed and desynchronised. It also suggested that midazolam pharmacokinetics were not influenced by the light-dark cycle in children in intensive care xmits (ICUs). 

A 21-day-old male weighing 2715 g received a dorsal penile block for elective circumcision. He received two 1-mL doses of 0.5% bupivacaine spaced apart by 2-3 min. Within minutes of the injections, the infant became lethargic and showed disconjugate gaze, intermittent exotropia and altered consciousness with hypotonia upon arrival to the emergency department. A serum sample revealed a bupivacaine concentration of 0.76gg/mL, which is consistent with bupivacaine toxicity. At 22 h after the event the infant fully recovered in the paediatric intensive care unit without any sequelae.

The variation in the error rates was probably due to the differences in the definitions of medication errors, the methodologies used and the settings. For example, the error rate in a neonatal intensive care unit is much higher than that in a general paediatric ward. Things could have gone better if there had been protocols for the management of status epilepticus (there were none on the ward). Double cover of busy neonatal and general paediatric units still goes on and should cease entirely. Intensive care availability has improved but needs to...
