The most commonly abused prescription drugs are opioids and opiates such as oxycodone and morphine, central nervous system depressants such as barbiturates and benzodiazepines, and stimulants such as dextroamphetamine and methylphenidate. Brand-name painkillers such as Vicodin and OxyContin, depressants such as Valium and Xanax, and stimulants such as Ritalin and Dexedrine are commonly abused (as are some OTC cough remedies). Although helpful and safe when used appropriately, these drugs can cause serious harm when taken in unapproved ways. [Pg.61]

Among the compounds that fall within this class are hydrocodone (e.g., Vicodin), oxycodone (e.g., OxyContin—an oral, controlled-release form of the drug), morphine, fentanyl, codeine, and related medications. Morphine and fentanyl are often used to alleviate severe pain, while codeine is used for milder pain. Other examples of opioids prescribed to relieve pain include propoxyphene (Darvon) hydromorphone (Dilaudid) and meperidine (Demerol), which is used less often because of its side effects. In addition to their effective pain-relieving properties, some of these medications can be used to relieve severe diarrhea (for example, Lomotil, also known as diphenoxylate) or severe coughs (codeine). [Pg.234]

Semisynthetic and synthetic narcotics are also produced that have opiate-like effects these narcotics are collectively known as opioids. They include methadone and the designer drug fentanyl, and a number of commonly prescribed medicines such as Darvon, Demerol, Dilaudid, Orlaam, OxyContin, Percodan, Talwin, and Vicodin. [Pg.389]

In the United States and other developed countries, illicit opium derivatives such as heroin or licit synthetic opioids such as Vicodin have generally replaced the use of smoked or eaten opium. According to Drug Abuse Warning Network (DAWN) data provided by the Substance Abuse and Mental Health Services Administration (SAMHSA), there were more than 82,000 emergency department admissions for narcotic analgesics/narcotic analgesic combinations in 2000. The prototypical opioid is morphine (A.137) (Figure A.39). Isolated in a crude form, called opium, morphine has been recognized as a potent pain killer for thousands of years. Although effective, morphine has a low oral availability (F = 25%). Two common derivatives of morphine include hydrocodone (Vicodin, A.138) and oxycodone (A.139), both of which have oral availabilities of greater than 75%. Oxycodone is often sold in an oral continuous-release form under the trade name of OxyContin. Not all opioids are semisynthetic derivatives of morphine. Dextropropoxyphene (Darvon, A.140) and tramadol (Tramal, A.141) are fully synthetic opioids. Both compounds preserve the pharmacophore of morphine as described in the morphine rule (see Chapter 11). Dextropropoxyphene and tramadol are depicted in Figure A.39 to highlight possible pharmacologically active conformations that resemble morphine. [Pg.380]

Heroin was first synthesized from morphine over a century ago. Since then, it has become one of the most abused substances. Research into why it produces such powerful effects has led to the discovery of specific opiate receptors and endogenous opioids (enkephalins and endorphins). These peptides appear to be neurotransmitters involved with the sensation of pain and pleasure. A number of opiates and synthetic opioids are available and can lead to dependency, including morphine, heroin, propoxyphene (Darvon), methadone, meperidine (Demerol), pentazocine (Talwin), hy-dromorphone (Dilaudid), oxycodone (Percodan), and hydrocodone (Vicodin, Damason-P), and codeine. [Pg.134]

Hydrocodone (trade name in Germany Dicoditfl, though marketing approval has been withdrawn due to addiction risk) is an opioid as well, with a pharmacology that is comparable to codeine. Particularly pronounced is its antitussive activity. Hydrocodone has higher potency than codeine, and is therefore used for moderate to severe pain. It is formed by palladium- or platinum-catalysed double bond isomerisation of codeine. Hydrocodone production has seen an almost constant upward trend over the past 20 years, teaching 56 tonnes in 2011. Other sustained-release opioids it will probably be approved as a more controlled schedule II narcotic, which may influence its prescription and use.
