Nonstandard treatment

Data from the peer-reviewed pilot SCDS of 217 children assessed at 5.5 years (Myers et al. 1995) are also considered in this chapter. The majority of today s turbines arc fueled wth natural gas or No. 2 distillate oil. Recently there has been increased interest in the burning of nonstandard liquid fuel oils or applications where fuel treatment is desirable. Gas turbines have been engineered to accommodate a wide spectrum of fuels. Over the years, units have been equipped to burn liquid fuels, including naphtha various grades of distillate, crude oils, and residual oils and blended, coal-derived liquids. Many of these nonstandard fuels require special provisions. For example, light fuels like naphtha require modifications Co the fuel handling system to address high volatility and poor lubricity properties. 

El Sayed and Nelson reviewed 42 trials between 1963 and 1993 including six trials prior to 1965 and one trial using razoxane, a nonstandard chemotherapy agent (73). A drawback to the El Sayed analysis is the inclusion of a single trial that had been accounted for twice because of its publication in two different journals. The addition of chemotherapy to locoregional treatment added an absolute benefit of 4.0% absolute benefits of concomitant chemotherapy were 8% (neoadjuvant and adjuvant chemotherapy were not assessed). 

White spot lesions vary from person to person, from tooth to tooth and from surface to surface, as well as with age of the lesion. Their nonstandard nature makes analysis of the lesion incredibly difficult. In order to increase the level of control over experiments with lesions, a standard method of producing in vitro lesions has been used in this study. Many of the features of white spot lesions can be mimicked by in vitro lesions, though no method completely replicates the development of the natural lesion. The lesions produced are usually referred to as caries-like lesions indicating that they are not a natural white spot lesion, but an artificially produced lesion for experimental analysis. These artificial lesions provide invaluable information about the formation, the processes of demineralization and remineralization, and the composition of the lesion at different stages, in addition to being the basis for the understanding of possible treatments of the lesion. 

Doctors may be reluctant to join a scheme that limits their ability to use nonstandard diagnostic tests and treatment protocols. 

Binary outcome data, or endpoints with exactly two possible outcomes, are commonly collected during drug development. Examples of binary (also referred to as dichotomous) endpoints include cure versus lack of cure of a disease or condition with treatment, relief versus lack of relief from symptoms, eradication versus persistence of an organism, presence versus absence of a medical outcome, and appearance versus lack of appearance of an adverse event. Binary data are a subset of what are termed discrete or categorical endpoint data. Sheiner and Beal (1) have referred to such endpoints as odd-type data, in that they are noncontinuous and therefore require the use of nonstandard methodology for proper analysis and interpretation.

Loading dose. A nonstandard and larger than usual dose given at the beginning of a course of regular treatment in order to shorten the time until the concentration is within the therapeutic window. 

Acidizing treatments are used to stimulate production by the acidic dissolution of undesirable materials on the walls of the producing formation or by pumping acid into the formation to improve permeability. Hydrochloric acid (HCl) is used for limestone formations. Hydrofluoric acid (HF) is added to the HCl for silicate formations. Inhibitors are added to the acid to protect steel tubing during the short period of exposure to acid during injection, and the following period when partially spent acid is returned to the surface. Suppliers use a variety of nonstandardized tests for acid inhibitor evaluation. 

Both liquid/gaseous ammonia and metaUic sodium are widely used in industry, but not for treatment of hazardous wastes. A Subtitle X permit is required under RCRA for processes that treat wastes regulated as hazardous in nonstandard ways. (Standard treatment processes, such as incineration and tank treatment, are permitted under other RCRA subtitles.)...
