An additional example of a bioavailability-predicted absorption plot is shown for a series of calcium antagonists (Fig. 19.8). Again there is considerable scatter in the data, and the four compounds - felodipine, nisoldipine, diltiazem, and verapamil -are predicted to be much better absorbed than was actually observed. Some of these compounds are known to undergo rapid first-pass metabolic clearance, and are also P-gp inhibitors or substrates (diltiazem and felodipine are P-gp substrates nicardipine and nitrendipine are P-gp inhibitors [25] verapamil is a P-gp inhibitor), and this might contribute to the scatter obtained in the graph. 

Calcium antagonists are able to affect nitric oxide production and suppress the peroxyni-trite-induced damage. Thus, nifedipine enhanced the bioavailability of endothelial NO in porcine endothelial cell cultures supposedly through an antioxidative mechanism [288], Pretreatment with nisoldipine, a vascular-selective calcium blocker of dihydropyridine-type, of confluent bovine aortic endothelial cells suppressed the peroxynitrite-induced GSH loss and increased cell survival [289]. 

A//SOLD/P/A/E. Administer nisoldipine orally once daily. Avoid administration with a high fat meal. Avoid grapefruit products before and after dosing. Swallow whole do not bite, divide, or crush. 

Lactation Discontinue nursing while taking amlodipine, diltiazem, nicardipine, verapamil, or nimodipine. If using felodipine, isradipine, nifedipine, or nisoldipine, decide whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother. 

Children Safety and efficacy of oral verapamil, diltiazem, felodipine, amlodipine, nicardipine, nifedipine, nisoldipine, and isradipine have not been established. 

Bioavailability of felodipine is not affected by food, but increased more than 2-fold when taken with doubly concentrated grapefruit juice vs water or orange juice. High-fat meals and grapefruit juice with nisoldipine should be avoided. 

Drugs that may affect fluconazole include cimetidine, hydrochlorothiazide, and rifampin. Drugs that may be affected by fluconazole include alfentanil, benzodiazepines, buspirone, carbamazepine, cisapride, oral contraceptives, corticosteroids, cyclosporine, haloperidol, HMG-CoA reductase inhibitors, losartan, nisoldipine, phenytoin, protease inhibitors, rifabutin, sirolimus, sulfonylureas, tacrolimus, theophylline, tolterodine, tricyclic antidepressants, vinca alkaloids, warfarin, zidovudine, and zolpidem.

For a few compounds claims have been made that they may even be selective for a particular vascular bed. Examples are nimodipine (cerebral vessels), nisoldipine (coronary arteries), and manidipine (renal vascular bed). Although potentially attractive, the clinical evidence for such a selectivity is so far not convincing.
